During a period of 421 days a total of 1,441 X-irradiated mosquitoes inoculated attenuated sporozoites of Burma (Thau.) strainPlasmodium falciparuminto a volunteer. Strict ethical guidelines were followed. The volunteer was also exposed at various times to nonirradiated mosquitoes carrying infective sporozoites of the same strain and of strains from Malaya, Panama and the Philippines, and did not develop falciparum malaria. Following exposure to nonirradiated mosquitoes carrying Chesson strainP. vivax, he developed vivax malaria. Antisporozoite antibody, demonstrated by the circumsporozoite precipitation test, appeared in his serum and reacted equally against the Burma strain ofP. falciparumand strains from Malaya, Panama and the Solomon Islands, but did not react with Chesson vivax sporozoites.

Drug overdosage, excluding narcotics, accounted for 1. 1 per cent of 129,504 adult admissions to a large municipal hospital during a 50-month period. Coma in 208 drugintoxicated patients was analyzed according to type of drug(s) ingested and efficacy of therapy with conservative measures and dialysis. Virtually all morbidity and 20 of 21 deaths occurred among the 58 patients presenting in stage IV coma. Specific therapy included diphenhydramine for phenothiazine-related extrapyramidal reactions; naloxone for respiratory depression; vasopressors for hypotension; neostigmine for amitriptyline-induced cardiac arrhythmias; NaHCO3and mannitol for osmotic diuresis and alkalinization of the urine. Barbiturates and glutethimide were the most frequent drugs ingested in all age groups. Hypotension occurred with every drug group encountered, but only in barbiturate coma did blood pressure consistently increase following fluid administration. Vasopressors were required for all other drug groups. Hemodialysis shortened the duration of barbiturate-induced coma but was of no benefit in glutethimide intoxication. Virtually all patients not in stage IV coma could be managed with conservative therapy, which included specific pharmacologic agents. Hemodialysis should probably be reserved for patients in stage IV coma who have ingested a dialyzable drug(s) and fail to improve with conservative management.

The purpose of the present study was to determine if serotonin alters human thyroid function. Fifteen patients with carcinoid tumors underwent clinical and laboratory evaluation of thyroid function. Ten of the subjects had excessive serotonin levels with the carcinoid syndrome; five of the subjects had normal serotonin levels. Five subjects also underwent monthly reevaluation for from five to 17 months after the initial evaluation. On initial evaluation 47 per cent of the patients had an abnormality of at least one of four tests of thyroid function (serum thyroxine, serum triiodothyronine, triiodothyronine resin uptake, or free thyroxine index). These abnormalities were found in patients with and without the carcinoid syndrome. Despite these abnormal laboratory tests, all of the patients were clinically euthyroid, and none had goiters. Although there is a high incidence of abnormal thyroid function tests in patients with carcinoid tumors, these alterations can not be attributed to serotonin.

Eleven patients with upper gastrointestinal problems and normal upper gastrointestinal roentgenograms were found to have hemorrhagic duodenitis unassociated with peptic ulcer disease. The total number of patients evaluated by fiberoptic esophagogastroduodenoscopy was 135, with a frequency of 12 per cent for hemorrhagic duodenitis. Alcohol, aspirin and other irritants may be contributory factors in some of these patients.

Cortisol premedication enabled the identification of minor variations in the rapidity of disposal of intravenous glucose in a group of 39 diabetes-prone women. Fifty-four per cent showed some abnormality by the criteria used. After intravenous glucose, control subjects responded with a brisk rise of plasma insulin, falling away rapidly to near fasting levels at 60 minutes. After steroid premedication fasting plasma insulin was raised, and after a brisk response fell more slowly and was well above the fasting level at 60 minutes. With increasing carbohydrate intolerance, the plasma insulin response became successively less brisk and more sustained, and cortisol premedication accentuated these changes. In women with apparently normal carbohydrate tolerance, those more than 15 per cent overweight had a significantly greater plasma insulin response to intravenous glucose, but this was not evident after cortisol premedication.

Serratia marcescensis a potential pathogen particularly for debilitated patients and for those in intensive care units (ICU's). In July 1972 there was an unexpected increase in the number ofSerratia marcescensisolates from patients in the Bellevue Hospital Center Pediatric ICU. During that single month nonpigmented Serratia was recovered from seven of 20 infants and children at risk; five of the seven died and Serratia contributed to the death of four. Consideration was given to closing the unit until more detailed characterization of the seven strains (on the basis of sensitivity to antibiotics, serotype and sensitivity to normal serum) indicated that each of the first five isolates was different. The unit was not closed and with continued surveillance there was a spontaneous decrease in the frequency of recovery of Serratia.

Serum and urine concentrations were determined following the intramuscular administration of 50 mg/M2tobramycin and gentamicin sulfate to ten patients. The highest mean serum concentration was obtained at 30 minutes after the injections and was 4.6. mg/ml for tobramycin and 5.2 mg/ml for gentamicin sulfate. The serum half-life was 2.1 hours for tobramycin and 2.3 hours for gentamicin sulfate. About 60 per cent of the administered dose of tobramycin was excreted in the urine within six hours. Tobramycin also was administered at a dose of 50 mg/m2intravenously every six hours to seven patients. The highest serum concentration on day 7 was 6.0 mg/ml. There was evidence of some accumulation of drug during multiple dosage studies.

During a two-and one-half week period, 12 of 55 (22 per cent) blood cultures submitted to the laboratory from eight patients on one inpatient ward were positive for an unusual organism,Moraxella nonliquefaciens. On the basis of initial bacteriologic survey in whichM. nonliquefacienswas found contaminating one of three culture tube holders in use on the ward, it was assumed that one of the four patients, from whom the 12 positive cultures were obtained, was the primary source of the organism. However, further investigation revealed that 14 of 31 (45 per cent) of the tube holders from the blood drawing area of the main laboratory and 5 of 41 (12 per cent) of the tube holders from the uninvolved inpatient wards of the hospital were contaminated with the same organism. That the tube holders were the immediate source of blood culture contamination was demonstrated by the ability to contaminate culture medium when contaminated tube holders were used to hold blood-drawing needles during simulated blood drawing. Although this widespread bacterial contamination could not be attributed to any of the involved patients, no other primary source was found. After establishment of adequate control measures, no contaminated tube holders or blood cultures positive forM. nonliquefacienswere observed

This case report presents a compromised host who had repeat lumbar punctures and developed aCandida albicansmeningitis which at necroscopy was localized to the area of the spinal cord where repeated lumbar punctures were done. The importance of aseptic technique is stressed in the performance of lumbar punctures.
