Clinical staging of maturation in 487 white (232 male) and 239 black (166 male) youths was compared with serum gonadotropin levels. Gonadotropin patterns were similar in both male groups. Luteinizing hormone (LH) rose progressively with maturation. It is uncertain if LH plateaus in pubertal males. Mean follicle stimulating hormone (FSH) titers plateaued by mid to late puberty in both races. In both female groups mean LH concentrations reached a plateau by midpuberty. No female (more than 65 were menstruating) had LH titers in the range found during midcycle surges of adult women. FSH average titers in the white females rose until midpuberty and subsequently plateaued at lower concentrations. In contrast, in the black females there were no statistical differences between mean FSH titers at all pubertal stages. Pubertal black females seemed to have higher FSH titers during maturation than did pubertal white females. There was considerable overlap of ranges of both gonadotropins, in both races, in both sexes, at all stages of maturation. Except for LH in males, gonadotropins plateaued during puberty, suggesting that titers of FSH in both sexes and LH in females reach adult levels before full maturation.

Tobramycin, a new aminoglycoside, was utilized in the treatment of 82 infectious episodes in 73 cancer patients. Forty-four episodes (54 per cent) responded to this antibiotic. Pneumonia and septicemia were the most common infections treated (62 per cent) and the response rates were 47 and 33 per cent, respectively. Tobramycin was most effective against the 11 urinary tract infections, producing a 91 per cent response rate. Gram-negative organisms were responsible for 92 per cent of the identified infections and 58 per cent of these infections responded to tobramycin.Klebsiellaspp.,E. coli, andPseudomonasspp. were the most common gram-negative bacilli causing infection. Sixty per cent ofKlebsiellainfections responded to tobramycin, compared to only 36 per cent of E. coll and 40 per cent of Pseudomonas infections. Only 24 per cent of infections occurring in patients with an initial neutrophil count of less than 1007mm3responded, whereas 69 per cent of infections occurring in patients with more than 1000 neutrophils/mm3responded to tobramycin. Nephrotoxicity was the only side effect of tobramycin and occurred during 17 per cent of the courses of therapy. However, in only 6 per cent was the drug exclusively responsible for the nephrotoxicity, Tobramycin has a spectrum of antibacterial activity similar to gentamicin and appears to be as effective in the therapy of gram-negative infections. This drug may be less nephrotoxic than gentamicin.

Considerable controversy still exists whether the cardiovascular manifestations of hyperthyroidism are due to the direct action of an increased amount of thyroid hormones on the heart or due to increased adrenergic activity of catecholamines. In this study, electrocardiograms (ECG) were recorded in 15 patients with hyperthyroidism before and after treatment with reserpine and propranolol alternately for a period of two weeks each. These electrocardiographs changes were compared with the ECG recorded in 13 patients who were rendered euthyroid with specific antithyroid treatments. The pretreatment ECG changes consisted of increased heart rate, an increase in height of P and R waves in Lead II, an increase in the total voltage of S1and R5, and an increase in the height of maximum R wave in V leads. All these abnormal ECG changes reverted to normal when the patients became euthyroid after specific antithyroid therapy, but they were not in any way corrected by adequate adrenergic blockade with either reserpine or propranolol except for partial reduction of rate. It is therefore postulated that sinus tachycardia in hyperthyroidism is due to the combined effects of an increased amount of thyroid hormones and increased adrenergic activity of catecholamines on the heart whereas the other abnormal ECG findings in hyperthyroidism are the results of direct action of increased amount of thyroid hormones on the heart.

The results of therapy with cytosine arabinoside as a single chemotherapeutic agent in 31 consecutive patients with acute granulocytic leukemia receiving 40 courses of therapy are reported. The initial complete remission rate in the evaluable course of therapy was 46 per cent. The morphological classification of the leukemia, age of the patient, and rate of deoxyribonucleic acid (DNA) synthesis by the leukemic leukocytes in tissue culture were correlated with the rate of complete remission. Maintenance therapy with 6-thioguanine did not significantly increase the length of initial remission in this series.

This report describes a patient with unusual ophthalmologic complications of Graves' disease. At the age of 56, the patient developed retinal vein thrombosis, and one year later optic atrophy of the other eye. Subsequently he developed the more commonly described signs of Graves' ophthalmopathy, including exophthalmos, stare, lid lag, and chemosis. The findings in this patient suggest that retinal vein thrombosis may occur in Graves' disease, and that optic neuritis may develop in the absence of more obvious signs of ophthalmopathy.

A young man who presented with pulmonary hemorrhage and iron deficiency anemia developed focal proliferate glomerulonephritis with linear deposits of immunoglobulin along the glomerular basement membrane (GBM) (Goodpasture's syndrome). Renal function deteriorated rapidly during six weeks of steroid therapy. Administration of nitrogen mustard was followed by a prompt and dramatic return of renal function to normal levels. Therapy with a rapidly acting cytotoxic agent may prevent progressive renal failure in anti-GBM antibody-mediated glomerulonephritis when given before glomerular destruction has occurred.
