Sera from 32 consecutive patients with systemic lupus erythematosus were examined for antiheart antibodies by an immunofluorescent technique. Results were compared to those obtained with sera from five control groups of patients. Sixty-three per cent of the patients with systemic lupus erythematosus had antiheart antibodies. Antibodies were present in equal frequency in those with and those without clinical cardiac disease and were found in six of seven patients with congestive heart failure. Although both subsarcolemmal and sarcolemmal patterns of immunofluorescence were seen, the former was predominant. The clinical significance of this high prevalence of antiheart antibodies in systemic lupus erythematosus is not clear. They may represent an immunologic response to release into the circulation of altered myocardial antigens.

The observation that calves with brisket disease and markedly reduced cardiac output often also have marked reduction in resting oxygen consumption stimulated comparison of oxygen consumption index in cardiac patients having normal and severely reduced cardiac indices. Group I consisted of 29 patients whose resting cardiac index was ≤1.5 liters/min/M2(mean:1.4 ± 0.1 standard deviation). Group II consisted of 100 cardiac patients selected only for cardiac index between 3.0 and 4.0 liters/min/M2(mean:3.3 ±0.3). The resting oxygen consumption index in Group I was 115.9 ± 17.3 ml/min/M2compared to 144.5±15.6 in Group II (p<.001). Similar reductions in oxygen consumption index have been observed in dogs with experimental hemorrhagic shock. Oxygen is the most nearly flow-limited of all physiologically important constituents of the blood in man. As cardiac index decreases, oxygen consumption may initially be maintained by increased oxygen extraction and compensatory changes in the oxygen-carrying capacity of blood and position of the oxyhemoglobin dissociation curve. With very severe reductions in cardiac index these mechanisms may become inadequate. Reduced oxygen consumption in patients with severely reduced cardiac index may represent obligatory adjustment at the tissue level to critically reduced oxygen delivery.

Bretylium tosylate, an anti-arrhythmic agent, has been shown in vitro to have positive inotropic effects on the heart. To evaluate the drugin vivo, eight dogs were studied during chloralose and morphine anesthesia at constant heart rates. Left ventricular and aortic pressures were measured during biplane ventriculograms. Measurements were made during a control period and 45 minutes after the intravenous administration of bretylium, 5 mg/kg. Left ventricular dP/dt was significantly increased from 3090 to 3540 mm Hg/sec (p<.01) while no significant change was observed in aortic or left ventricular end-diastolic pressure. In addition, no significant change was observed in end-diastolic volume, stroke volume, ejection fraction, or pressure-volume work. From the force velocity relation, the maximum velocity of contractile element shortening increased in three, decreased in one, and was unchanged in four animals. Within seven minutes of the administration of the bretylium, five animals developed a variety of arrhythmias, including sinus, nodal, and ventricular tachycardia; ventricular bigeminy; and second degree arteriovenous block. These arrhythmias abated within 15 minutes and were felt to be consistent with the immediate release of catecholamines. Although individual animals showed marked effects on ventricular performance 45 minutes after injection, no consistent positive inotropic effect was noted in this series.

Serum lysozyme was related to tuberculosis disease activity in 67 patients with proven active pulmonary tuberculosis. A strong relationship was established between serum lysozyme activity and the clinical classification of disease. Relationships were poor between serum lysozyme and the presence or absence of tubercle bacilli in the sputum. The only two patients with lysozymuria died. Elderly patients with tuberculosis had significantly higher serum lysozyme activity than did younger subjects with comparable amounts of pulmonary tuberculosis. The clinical usefulness of serum lysozyme determinations in patients with pulmonary tuberculosis, however, must await further investigation. Evidence is presented to show that chronic tissue inflammation is the most likely source of the increased serum lysozyme activity in patients with active pulmonary tuberculosis.

The effect of histamine and secretin infusions on gastric secretory volume and acid production has been studied in 22 patients with peptic ulcer disease and one normal volunteer. Some of these patients had normal mucosa; some had gastric mucosal hypertrophy and/or antritis. During histamine infusion the mean pH was 1.14 and the mean secretory volume was 351 cc/hr. With histamine-secretin infusion these values rose to 3.51 and 584 cc/hr, respectively. Duodenal regurgitation was excluded by appropriate studies in seven studies. Patients infused with histamine alone for four hours showed no change in pH or secretory volume. Five patients had secretin infusion only. During the basal period their mean pH was 4.24 and mean secretory volume 101.2 cc/hr. During the period of secretin infusion these values rose to 8.14 and 278.6 cc/hr, respectively. Statistical analysis of these findings suggests that the production of an alkaline fluid during the secretin infusion is related to antritis. These findings differ from previous studies in which secretin was given either alone or in combination with histamine infusion. The clinical or pathological significance of these observations is discussed.

Five patients with hypothalamic hypothyroidism have been identified. Associated endocrine deficiencies, including diabetes insipidus, low gonadotrophin levels, and decreased pituitary reserve for adrenocorticotropic hormone and growth hormone, were uncovered in three of the five. All showed a prompt rise in thyrotropin following infusion of thyrotropin-releasing hormone, but no rise in triiodothyronine levels. These findings suggest that chronic understimulation of the thyroid by thyrotropin occurs in hypothalamic hypothyroidism, leading to thyroidal failure.

The testicular feminization syndrome is a variety of male pseudohermaphroditism with characteristic clinical, biochemical, and genetic findings. Three affected sibs are presented. One patient developed a metastatic seminoma and a second patient had a testicular tubular adenoma (Sertoli cell tumor). The pertinent literature is reviewed, and comments regarding treatment are made emphasizing the increased incidence of gonadal malignancy in these patients.

Two patients with septic thrombophlebitis with protracted, antibiotic-resistant septicemia due toKlebsiellaspecies following the use of plastic indwelling venous cannulas are reported. One patient was remarkable in that septic microabscesses were found in the wall of the vein. The clinical and pathological features of septic thrombophlebitis are discussed, in particular the refractory septicemia, the absence of obvious signs of infection in the involved vein, and the significance of the intramural microabscess. This latter may explain the septicemia unresponsive to antimicrobial therapy. Early diagnosis of this entity and the necessity for surgical removal of infected thrombosed veins are stressed.

A patient in whom profound granulocytopenia developed during the course of infectious mononucleosis was observed. Arrested development of bone marrow granulocyte precursors was seen. These findings and possible etiological factors are discussed.
