The frequency ofPscudomonassp. in the stools and throats of 87 patients with leukemia was determined. Twenty-five percent of the patients were already carriers on admission to the hospital and a total of 54% of the patients became carriers. The majority of patients who acquiredPscudomonassp. while in the hospital, did so during the first four weeks. Twenty-eight carriers received antipseudomonal antibiotics parenterally which eliminated the organism from the stool or throat of 32% of them. The frequency ofPscudomonassp. infection was 26% in the carriers compared to 13% in the non-carriers. In the majority of infections occurring in carriers, the same pyocine type ofPseudomonassp. was found at the site of infection as was present in the stool or throat swab specimens.

Before gentamicin became available mortality among 82 patients with primary gram-negative pneumonias treated at this medical center (1963 to 1965) was 49% (40 deaths). In this experience eight of ten patients with nonbacteremic pseudomonas pneumonias died. In 1968 to 1969 gentamicin was used as the single antibiotic in 16 consecutive patients with primary gram-negative pneumonias. Ten were due toPseudomonas aeruginosa, three to enterobacter, two toEscherichia coli, and one was a mixed klebsiella-proteus infection. Gentamicin was given intramuscularly for eight to 11 days at a dosage of 1.5 to 3.0 mg/kg a day in three divided injections. The maximum amount of aminoglueoside administered was 1.98 g. All organisms were sensitive to gentamicin and the drug was well tolerated. Mortality among these patients was 31% (five patients). Every death occurred in patients with pseudomonas pneumonias, and their mortality was 50% (five of ten patients). Three patients with pseudomonas pneumonias who recovered had failed to improve after full courses of colistin. Of the generally available medicines today, gentamicin appears to be the antibiotic of choice in pseudomonas,Excherichia colior enterobacter pneumonias.

In experiments on ten trained dogs, the previous observation of increased myocardial hypersensitivity to catecholamines during the acute postinfarction period was confirmed. In addition, a similar acute hypersensitivity to angiotensin and vasopressin was demonstrated. Long-term follow-up of animals with multiple infarets revealed a chronic hypersensitivity to these four agents. The potential clinical significance of these findings is discussed. By creation of a chronic myocardial hypersensitivity state, a more suitable experimental model is offered for long-term studies of the damaged, hypersensitive myocardium.

Despite valid criticisms against the indiscriminate use of various drugs in the treatment of obesity, little scientific evidence is available concerning the value of relatively small amounts of thyroid hormone. Therefore, the effects of the addition of three grains of desiccaled thyroid to 15 mg of a sustained-release preparation of dextroam-phetamine sulfate have been examined in 101 obese patients in a controlled, double-blind study. This addition produced a greater weight loss than amphetamine alone, which, though slight, was statistically significant, without any clinical features of thyroid hormone excess. On the basis of experimental evidence, the increased weight loss is interpreted as a pharmacologic effect of a slightly greater than physiologic dose of thyroid hormone.

Sixty-three patients were investigated for the etiology of fever appearing thuring acute alcohol withdrawal. Thirty patients experienced a withdrawal syndrome that was limited to the acute tremulous state. There were nine episodes of fever in eight (27%) of them. The incidence of fever among these patients was no greater than among a control group of non-alcoholic patients admitted during the same period. Thirty-three patients experienced delirium tremens. There were 28 episodes of fever in 27 (82%) of these patients; 14 had fever secondary to pneumonia. Dehydration was the apparent cause of seven febrile episodes and seven other episodes were of diverse etiology. Fever during alcohol withdrawal usually appears in the hospital at the height of the withdrawal syndrome. Bronchopneumonia and dehydration are the mose common and remediable causes of fever.

Interaction of platelet surface with collagen and with other platelets is vital to initiation of hemostasis. The surface of the early hemostatic reaction was studied by scanning electron microscopy of white and red mural thrombi in iliac arteries of anesthetized rats and correlated with transmission electron microscopy (TEM) of internal structure of similar thrombi. One minute after vessel injury many platelets were seen covering the endothelial break in white thrombi. Platelets were flat or swollen and most possessed dendritie processes which often blended with collagen strands. No fibrin was evident and pre-treatment with heparin did not change appearance of thrombus. TEM showed platelets apparently adhering to ruptured endothelium and subendothelial collagen at the site of injury. Five minutes after injury the surface of the white thrombus was more homogenous and further raised from the endothelial surface. A small amount of fibrin was present at the periphery of the break. Reduction of blood flow resulted in a red thrombus with a surface that had a few platelets and many red blood cells (RBC) held together by fibrin strands. These studies indicate that early hemostasis in high flow vessels begins by adherence of platelets to ruptured endothelium and suhendothelial collagen followed by adherence of other platelets to initial platelets and morphologically normal endothelial surface. Fibrin formation and RBC appear unimportant at this stage of white thrombus formation, but are integral to the early states of the red thrombus.
