Invited Review: A patient with primary hypothyroidism was given thyrotropin releasing hormone (TRH) intravenously prior to commencement of replacement therapy and again while receiving 25, 50 and 100 micrograms of 1-thyroxine per day. Initially, basal levels of thyrotropin were markedly elevated and rose dramatically after TRH administration. With increasing doses of thyroxine, basal thyrotropin levels fell and response to TRH progressively diminished. Basal triiodothyronine (T3) levels were low on each occasion and failed to rise in response to TRH. However, the serum T3level when the patient was receiving 100 micrograms of thyroxine per day was 20 ng/100 ml higher than before therapy, probably as a result of peripheral conversion of thyroxine to triiodothyronine.

The dual aspects of medicine, its strength as an exact and aloof scientific discipline and its strong elements as a humanistic art of healing and life, are discussed in this paper in the light of the author's personal experience. The searching for a harmonious merging of moral and ethical concepts of medicine (altruism, deep involvements in human suffering, and compassion) with the principles of the exact scientific discipline must be made an integral part in the education of a modern physician, both early in life and in the medical school, on the wards of the hospital, and in the home of the sick. Humanism in medicine is essential today in view of the immeasurable loneliness of man in our times and the fears and anguishes which surround our lives in this technological age. The ennobling example of the lives and works of great physicians and the exploration of the truth of the heart in the world's immortal literature should be guides for the education of a modern physician.

The prevalence of coronary atherosclerosis in patients with valvular heart disease has been determined by selective coronary cine arteriography. Twenty eight per cent of 150 patients had angiographic evidence of some degree of coronary atherosclerosis; 101 of the patients were age 50 or older, and 38 exhibited coronary atherosclerosis. Thus 90 per cent of detectable atherosclerosis occurred in patients over 50 years of age and with nearly equal sex distribution. In those patients with coronary atherosclerosis nearly half had multivessel involvement and two thirds had a significant (>50 per cent) obstructing lesion. The sites of predilection and distribution were similar to other reported series. The prevalence of coronary atherosclerosis was not influenced by the site of valvular involvement even when differences in age and sex were considered. There was no relationship between chest pain and coronary arterial disease.

Phagocytic function was compared in nondiabetic control and 11 poorly controlled diabetic subjects before and after treatment. Prior to therapy, the rates at which the type-25 pneumococcus employed as the test organism was killed by whole blood were significantly reduced (p<.01) in the diabetic group. This delay in bacterial killing appeared to result from a marked reduction in phagocytosis, the degree of impairment correlating closely (p<.001) with fasting glucose levels. Both of these abnormalities were reversed by diabetic treatment.

Renal renin activity (RRA) of rats maintained on a low sodium diet for 14 or 17 days and given desoxycorticosterone acetate or depot desoxycorticosterone daily for the final seven or ten days was compared with RRA of rats maintained on low, normal, and high sodium diets for similar periods of time. There was no significant difference in sodium balance between the groups maintained on the low sodium diet or the low sodium diet plus desoxycorticosterone. Rats on the low sodium diet plus desoxycorticosterone acetate (DOCA) 2.5 mg daily for seven days had RRA lower than rats on the low sodium diet alone (p=.003). When the low sodium diet plus DOCA 10 mg daily was given, a further fall in RRA was observed (p=.007). No further fall in RRA was observed when the period of DOCA administration was increased to ten days. By contrast, rats given depot desoxycorticosterone 10 mg daily for seven days had RRA similar to that of rats maintained on the low sodium diet alone despite cumulative sodium retention identical to that of DOCA-treated rats. These results suggest that the pharmacologic doses of desoxycorticosterone attained with the rapidly absorbed acetate form (DOCA) but not with the poorly absorbed repository form of desoxycorticosterone can suppress RRA in the absence of significant sodium and fluid retention.

In 20 obese patients the glomerular filtration rate (GFR) was determined before and after 14 days' fasting. A significant reduction in GFR was observed in the patients of group I who received only waterad libitumand in the patients of group II, who received a NaCl supplement sufficient to maintain a positive sodium balance. In group III, receiving a daily supplement of 100 gm glucose only, the weight loss after two weeks was similar to that observed in group II; however, no change in GFR was observed. Sodium depletion, therefore, does not seem to be responsible for the drop in GFR observed in groups I and II. The reduction in GFR merits further investigations.

Serum vitamin B12and unsaturated B12binding capacity were measured in patients with a variety of hematological and nonhematological diseases. Both determinations were markedly increased in 25 patients with chronic myelogenous leukemia in relapse but decreased toward normal when clinical and hematological remission was achieved. However, values remained higher than normal in this disease even in its blastic phase and when treatment had rendered the marrow aplastic. Increases and decreases in the leukocyte count usually coincided with increases and decreases in the serum vitamin B12and binding protein values. The finding of an increased vitamin B12binding protein in patients with iron-deficiency anemia is unexplained.

Diazoxide has potent inhibitory effects upon vascular smooth muscle tone, insulin secretion, natriuresis, and uric acid excretion, suggesting that its receptor site is a biochemical pathway common to the maintenance of each of these processes. Following reports of dependence of hypotensive activity upon the rate of intravenous injection, it has been postulated that arteriolar diazoxide receptor sites possess unique properties in that they must be filled rapidly to produce sustained arteriolar dilatation. Three milligrams of diazoxide per kilogram body weight was administered intravenously over 30 minutes and subsequently over 30 seconds to ten hypertensives. Sustained blood pressure reduction, hyperglycemia, reduction of plasma insulin levels, and reductions of urinary water, electrolyte, and uric acid excretion were induced by this dose of diazoxide. However, no significant difference in any of these effects was demonstrable between phases of rapid and slow injection. For all important effects of diazoxide, the response appears dependent upon equilibrium of receptor sites with circulating drug concentration, and no special properties of these sites need be postulated.
