Risk factors for coronary artery disease, diabetes and hypertension were determined among 13,148 men participating in periodic health examination programs between 1950 and 1964. During the period of observation there were 401 new events of coronary artery disease, 241 new events of diabetes and 988 new events of hypertension based on the criteria used in this study. Important risk factors for coronary artery disease were elevated serum cholesterol and systolic blood pressure and hemoglobin values above 17 g%. For diabetes the important risk factors were blood sugar, family history of diabetes, elevated serum cholesterol and increased relative weight. For hypertension the factors were systolic blood pressure, increased relative weight, cigarette smoking, elevated serum cholesterol ad the diagnosis of diabetes. The effect of some of the risk factors varied with age. Elevated serum cholesterol was the one risk factor common to all three conditions.

The effect of circulatory congestion upon tolerance of orthostatic stress was evaluated in three groups of patients. Five patients with heart failure (group I) were studied hemodynamically in the supine and upright positions. They maintained cardiac index and stroke volume without changes in heart rate or peripheral vascular resistance. Six normal subjects (group II) were subjected to orthostatic stress before and after volume expansion. Before expansion, they responded to orthostatic stress with decreases in cardiac index (-22%) and stroke volume (-35%) and an increase in heart rate (+29%). Arterial pressure was maintained by an increase in peripheral vascular resistance (+43%). The arteriovenous oxygen difference widened by 52%. After expansion, the cardiac index did not change significantly, whereas heart rate increased by 6% and the arteriovenous oxygen difference widened by 26%; both changes were significantly smaller than before expansion. In order to determine if the increased tolerance of orthostatic stress associated with circulatory congestion would protect against circulatory deconditioning by bed rest, eight patients with heart failure (group III) were studied in the supine and upright positions after a period of bed rest. Orthostatic stress was well tolerated, without significant change in heart rate or systolic blood pressure. The diastolic blood pressure increased by 7 mm Hg, narrowing the pulse pressure slightly. In six patients restudied after ambulation, there was no significant difference between the bed rest and ambulatory responses. It therefore appears that circulatory congestion improves orthostatic tolerance in normal subjects and prevents deconditioning by bed rest in patients with congestive heart failure.

A 23 year old pregnant female wild uremia due to lupus nephritis was hemodialyzed twice a week from the 20th lo the 37th week of pregnancy, in an effort to sustain the life of the patient and to improve the chances of fetal survival. Kidney biopsy showed severe involvement of all glomeruli. By the 37th week, kidney function had markedly improved in spite of no change in the biopsy appearance, and dialysis was discontinued. The clinical improvement has been maintained postpartum. This experience suggests that chronic hemodialysis is a practical and safe procedure in pregnant patients. It further suggests that selected patients with severe lupus nephritis may be candidates for chronic hemodialysis. Finally, it is a striking example of the difficulties in deriving prognosis from the biopsy appearance in lupus.

A study was carried out on the serogroups ofPseudomonas aeruginosaisolated from patients with neoplasia and on the antibody response to the O antigens of the isolates of the subjects.P. aeruginosawas isolated from the urinary tract in 43% of the subjects, from the respiratory tract in 27%, from wounds in 20%, and from other sources in the remaining 10%. Serogroups II, III, and IV accounted for 73% of the isolates. Serogroup distribution was unrelated to basic disease or site of isolation. A specific antibody response was documented in 46% of patients with carcinoma, sarcoma, or other diseases, but in only 13% of subjects with leukemia, lymphosarcoma, Hodgkin's disease, or multiple myeloma.

Vancomycin is the drug of choice for the treatment of enterococcal endocarditis in those patients who can not tolerate penicillin. In vitro studies demonstrated marked synergism of the combination of vancomysin plus streptomycin for 18 of 20 strains of enterococci. Thus, it is suggested that streptomycin be used with vancomycin in treatment of enterococcal endocarditis in patients who can not tolerate penicillin.

Previous work has established that the activity of a heme-containing enzyme, catalase, is altered in erythrocytes in thatassemia minor and iron deficiency, two conditions associated with impaired production of hemoglobin. The present study was undertaken to determine (1) whether this effect on enzyme activity was limited to iron-containing enzymes and (2) whether enzyme activities in cells other than erythrocytes were also affected. Activity of erythrocyte nucleoside phosphorylase, an enzyme not requiring iron, was found to be increased in thalassemia minor and unchanged in iron deficiency. The activities of granulocyte catalase and myeloperoxidase, both heme-containing enzymes, were normal in thalassemia and mild iron deficiency. These findings suggest that the increases in activities of erythrocyte catalase and other enzymes in thalassemia minor are nonspecific, whereas the decrease in erythrocyte catalase activity in iron deficiency is related to the iron decrease. Furthermore, they suggest that the changes in crythrocyte enzymes in thalassemia or iron deficiency are not necessarily exhibited by changes in iron-containing enzymes in other cells.

Serum testosterone levels rise gradually in prepubertal males. A significant increase in circulating luteinizing hormone (LH) between ages 11 and 12 years precedes a sharp increase in testosterone which occurs between ages 12 to 14 years. Testosterone binding affinity (TBA) gradually decreases and is in the normal adult male range at adolescence.
