A variety of methods have been developed for the production of antibodies to gastrin. By previous techniques antibodies to gastrin have been shown to exhibit immunological cross-reactivity with cholecystokinin-pancreozymin. In this study antibodies to gastrin which exhibited no immunological cross-reactivity with cholecystokinin-pancreozymin were produced and characterzied. Rabbits were immunized with human gastrin I, residues 1-17, covalently conjugated to havine serum albumin molecules using triazine. Other rabbits were immunized with human gastrin I, resides 1-13, conjugated to the same carrier protein in the same manner. An additional group of animals received both immunogens sequentially. It was possible to substantially enhance the immunologic specificity of antibodies to gastrin by immunization with the amino tridecapeptide of human gastrin I following earlier immunization with intact human gastrin I. Following repeated immunization exclusively with the conjugate containing the amino tridecapeptide of gastrin, antibodies were produced and characterized which exhibited no immunological cross-reactivity with cholecystokinin-pancreozymin. These antibodies with exclusive specificity fot the gastrin molecule may be of particular value in specific identification and measurement of gastrin, particularly in those tissues and physiologic fluids in which substantial amounts of cholecystokinin-pancreozymin may be present.

Extragenital gonorrhea may present as a difficult and confusing differential diagnostic problem. Joint involvement, feeling or sustained, and vesiculopustular or hemorrhagic bullous skin lesions may initially be misinterpreted as manifestations of rheumatic or connective tissue disease. Within three years at the Bernalillo County Medical Center, five of sixty-four cases hospitalized and later proven by specific culture methods to have gonorrhea primarily exhibited migratory cutaneous and joint involvement. Initially in several of our patients the possibility of systemic gonococcal infection was never seriously entertained and was only clarified by return of positive cultures. Of interest was the apparent temporary improvement of joint complaints and constitutional systems in several patients initially treated with only bed rest and salicylates, again emphasizing the clinical difficulty of making a diagnosis of gonococcal sepsis if it is not considered during primary work-up. Review of literature and examination of the clinical course of our patients emphasize the importance of diagnostic awareness of these confusing manifestations of systemic gonococcal infection.

An 18 year old Negro primipara at term had an uneventful elective medical induction of labor. Six hours postpartum, shock and an acute surgical abdomen intervened. Exploration of the abdomen disclosed an acute hemorrhagic pancreatitis,Biochemical determinations revealed persistent hypercalcemia and hypophosphatemia consistent with a diagnosis of hyperparthyroidism. Subsequently, a chief cell adenoma of a parathyroid gland was resected. Her child exhibited tetany, hypocalcemia and hyperphosphatemia during the second week of life. An association between hyperparathyroidism and pancreatitis has been documented in the literature but has not here-to-fore been reported for the pregnant woman. Neonatal tetany is a recognized consequence of maternal hyperparathyroidism. The likelihood of hyperparathyroidism, therefore, should be considered in a pregnant woman whenever pancreatitis is diagnosed or whenever her child develops neonatal tetany.

Meningococci are extremely sensitive to coumermycin A1in vitro. The results of a controlled double-blind study in Naval recruits revealed, however, that this drug is ineffective in eliminating meningococci in nasopharyngeal carriers. Coumermycin A2in its present form is not recommended for use in the elimination of meningococci among carriers.

A patient is reported with features of three uncommon clinical entities, i.e. pseudohypoparathyroidism, pseudopseudohypoparathyroidism and basal cell nevus syndrome. The patient's short metacarpals and metatarsals, tetany, intermittent hypocalcemia and relative end organ insensitivity to parathyroid extract suggested pseudohypoparathyroidism. However, the usually normal serum calcium and phosphorus concentrations, in association with the physical defects of pseudohypoparathyroidism, suggested pseudopseudohypoparathyroidism. The features of basal cell nevus syndrome in the patient were jaw cysts, multiple nevi, strabismus, calcification of the falx cerebri, and a calcified pelvie mass. These observations strengthen previous contentions that these syndromes are variants of the same genetic defect. Following infusion of calcium intravenously, the return of serum calcium to normal preinfusion level was markedly delayed in this patient. This response was completely different from that previously described for pseudohypoparathyroidism. No comparable reports of this test are available in pseudopseudohypoparatbyroidism or basal cell nevus syndrome.

A case of glandular toxoplasmosis in a 38 year old farmer is described. The chief clinical features in this case were enlarged moderately tender axillary lymph nodes and an illness characterized by fatigue and lassitude lasting several months. The parasites were isolated from biopsied lymph nodes. Removal of a large mass of moderately tender lymph nodes from which the organisms were isolated coincided with an improvement of the patient's sense of well-being. The patient recovered without any specific treatment and remained well at the time of a five year follow-up examination.
