Daily intramuscular administration of 100 mg/kg of cephalothin and cephaloridine to rhesus monkeys beginning 16 hours post-intravenous challenge with a penicillin-resistant, phage type 80/81 staphylococcus, was followed by comparable results in most parameters measured. In reference to mortality, 11 of 12 untreated controls died as compared to 3 of 14 and 5 of 14 given cephalothin and cephaloridine, respectively. A new orally-absorbed cephalosporin, cephalexin, was compared to cephalothin and cephaloridine. Daily intragastric administration of 100 mg/kg was compared to the same dose of the two injectable preparations; 1 of 8, 3 of 8 and 2 of 8 given cephalothin, cephaloridine and cephalexin, respectively, died as compared to 7 of 8 deaths in untreated controls. These data suggest that the new orally-absorbed cephalosporin, cephalexin, was as effective as the parenteral preparations in the therapy of severe staphylococcal infection in monkeys.

Two patients in the non-ketotic hyperglycemic diabetic state are reported. Blood glucose levels of 1982 mg % and 660 mg % and concurrent serum immunoreactive insulin activities of five μu/ml and 12 μU/mi were found. It is postulated that these abnormally low insulin activities were the result of long standing hyperglycemic stress to the pancreatic beta cells with subsequent exhaustion of insulin reserves. Moreover, it is postulated that the massive hyperglycemia allowed for the utilization of a sufficient amount of glucose to prevent the development of ketoacidosis.

The combination of an acid-stable, penicillinase-resistant penicillin with an acid-stable penicillin having a significant gram-negative spectrum might provide all the basic attributes of an ideal “penicillin.” Detailed in vitro studies of such a combination with 78 gram-positive and gram-negative strains are reported. Antagonism was never observed. Synergism occurred against five of 17 strains ofS. aurcus, four of 13 strains ofE. coli, five of eight strains ofAcrobacter, seven of 18 strains ofKlebsiella, three of 12 strains ofProteus, three of seven strains ofPseudomonasand one of three strains ofH. influenzac. In relatively few of these strains was the degree of synergism marked. Overall, there was some increase in antibacterial activity provided by the use of the combination in 40 of the 78 strains tested, but there was no perceptible gain against many of the more resistant gram-negatives which currently present clinicians with their most difficult therapeutic problems. Consideration of the in vitro activity of the combination, in relation to usual attainable blood and urine levels, leads to the conclusion that manufacture of a combination capsule would offer relatively little therapeutic benefit. Widespread use of such a combination capsule might tend to discourage critical diagnostic thinking and the use of important laboratory aids and could lead to an increased frequency of superinfection with resistant organisms in susceptible hosts. On the other hand, the data suggest that selective utilization of the combination might prove beneficial in certain clinical situations.

Twenty-three of 78 examined members of a kindred totaling 108 persons show one or more features of a syndrome consisting of renal disease, hearing loss, prolinuria and ichthyosis. The 14 year old female proband and her 9 year old brother who are progeny of a consanguineous marriage (first cousins once removed), have nephritis, ichthyosis and prolinuria. A third sib has hematuria and renal cysts, while the father has nerve deafness and the mother has recurrent hematuria. Renal calculi, ichthyosis and neurosensory hearing loss are found in other family members. A tentative genetic hypothesis is that an incompletely dominant, pleiotropic, and not completely penetrant trait is segregating in the family. The parents, the mildly affected sib and the other affected relatives are regarded as heterozygous while the severely affected sibs are homozygous.

Five pairs of subjects were studied at bed rest for artificial acclimatization to heat in a climatic laboratory at Charity Hospital of Louisiana. Each pair included a patient with congestive heart failure (CHF) and a control subject without heart disease. All subjects were studied under identical environmental conditions. The duration of the studies varied from 8 to 18 days. Criteria for defining the acclimatized state are given and the results are presented and discussed. Except for the first two pairs of subjects, tolerance was limited to a maximal environment of 900F and 41% RH. In all five patients with congestive heart failure, tolerance to heat was much reduced as compared to control subjects and none of the patients with CHF showed evidence of acclimatization to the hot, dry environment (900F and 41% RH). The inability of patients with CHF to acclimatize to heat even at bed rest is discussed in light of certain cardiovascular physiologic changes required for heat acclimatization. It is concluded that such patients cannot depend upon the process of acclimatization while living in tropical or subtropical climates. Measures such as air-conditioning and other means of facilitating heat loss are necessary for lessening the cardiovascular burden and improving cardiac function.

Atrio-ventricular block occurred in nine of 29 patients undergoing rapid right atrial pacing as a diagnostic stress test for ischemic heart disease. Six developing block had coronary artery disease, while three had no evidence of heart disease. Varying PR intervals and periods of Wenckebach conduction were manifest in all nine; three patients showed periods of 2:1 atri-ventricular block. Atropine, 0.4 mg or less, was administered to four patients. 1:1 conduction was restored in three. In the other patient, with posterior coronary artery disease, Atropine reduced 2:1 atrio-ventricular block to Wenckebach conduction. Second degree atrio-ventricular block appears to be common during atrial pacing without other evidence for atrio-ventricular nodal disease, and does not necessarily indicate disease of the cardiac conduction system.

Sodium salicylate was administered intravenously to five obese nondiabetic subjects before and during a fasting period. Blood glucose and plasma nonesterified fatty acid levels were measured. There was no discernible difference in the effect of sodium salicylate on blood glucose values before and during the fast. Sodium salicylate administration during fasting resulted in a lowering of plasma nonesterified fatty acid below the baseline values, an effect that did not occur before the fasting period.
