Ventricular function assessed by means of biplane cineangiography requires the use of radiographic contrast media. The time related effects of these agents on the peripheral circulation and myocardial contractility must therefore he defined. Six mongrel dogs underwent biplane left ventricular cineangiography (sodium iothalamate, 80%), at a film speed of 60 frames per second, during chloralose and morphine anesthesia, for the determination of left ventricular volume and dimensions. Left ventricular pressure (SF-1 micromanometer), aortic pressure, and left ventricular dP/dt were simultaneously recorded. The force velocity relations for consecutive beats were calculated during the period of isovolumic contraction. Contractility (Vmax) was unchanged during the first five opacified beats and decreased 30% in the sixth and seventh beats in two animals due to the acute effects of the contrast medium. The animals were re-studied 30 minutes later without intervention to determine the residual effects of the initial study (Control vs. Control + Time). No significant differences were observed in the ventricular volume or pressure data although aortic pressure decreased strikingly in four animals. Vmaxwas the same in Control and Control + Time in five animals and decreased in the sixth from 4.85 to 4.40 cm/cm/sec. It is concluded that sodium iothalamate, 80%, does not effect myocardial contractility during the first four or five opacified cardiac cycles. However, a decrease in contractility was observed in two animals six and seven heals following the injection of the contrast medium with a return to the control status within a 30 minute study period.

During a three year period 07 cancer patients developedPseudomonassp. bacteremia. Fever greater than 101° was present in 97% of the patients at the onset of bacteremia. In 36 patients the lung was the site of origin. Ecthyma gangrenosum occurred in 28% of the patients. During the two week period prior to the onset of bacteremia 81% of the patients had received cancer chemotherapy and 58% had received adrenal corticosteroids. Only 14 patients (21%) recovered. The majority of the patients (69%) received a polymyxin antibiotic ns therapy for their infection and 24% of these patients recovered. Only 14% of the remaining 21 patients survived. Sixty-four percent of the patients whose absolute neutrophil count increased during the week after onset recovered compared to 8% of those whose neutrophil count decreased during that period. Polymyxin antibiotics have not improved the survival rate of patients withPseudomonassp. bacteremia.

Xylitol and glucose (500 ml 10% solution) were infused over 60 minutes into five healthy subjects. During the glucose infusion, growth hormone levels were low, but rose over the subsequent three hours. With xylitol, the rise in growth hormone occurred earlier mid maximal levels were attained during the hour following temination of the infusion. The levels of growth hormone were higher after xylitol than glucose. Although xylitol resulted in small changes in plasma glucose levels, it is unlikely that this accounted for the increase in growth hormone. It seems probable that xylitol or a metabolite directly stimulates growth hormone release.

In 38 male patients on chronic hemodialysis, plasma erythropoitic activity was correlated with hemotocrit and saturation of the plasma iron binding capacity (transferrin). Erythropoietic activity was absent in 15/16 patients transfused regularly for six mouths, and in all 14 patients transfused occasionally. Hemotocrits averaged 25 volume percent and the saturation of the iron binding capacity was 56 and 54 percent respectively. After reduction of the number of transfusions, erythropoietic activity was present in nine of 12 surviving patients in the regularly transfused group, as well as in seven of eight patients in a third group not transfused for at least six months. There was an excellent inverse correlation between decreasing hematocrit and saturation of the iron binding capacity on one side and increased erythropoietic activity on the other in most cases. In general, many patients after several months of hemodialysis without blood transfusions were not only able to tolerate lower hematocrits, but eventually increase their packed cell volumes following an increase in erythropoietic activity.

An abnormally large chromosome, larger than the largest chromosome of the complement, was seen in 72 (34%) metaphases obtained from the bone marrow mill peripheral leukocytes of three patients with clinical diagnosis of macroglobulinemia of Waldenstrom (MW). This abnormal chromosome was not an additional chromosome; rather it replaced one of the chromosomes in group A (chromosome 1, 2, or 3). This “abnormal” chromosome was seen in 58 (40%) metaphases obtained from two normal persons' leukocytes cultured with plasma obtained from patients with MW. Only five (10%) metaphases from one of the two control cultures had this “abnormal” chromosome. Thus, it appeared that this abnormally large chromosome may be induced by the abnormal protein rather than a feature of the basic disease.

It seems reasonable to assume that non-beta islet cell tumors of the pancreas can elaborate secretin as well as gastrin, since they share a common cell of origin with the antrum and the duodenum where these hormones lire normally produced. Two distinct clinical syndromes are now associated with non-beta islet cell tumors of the pancreas. Theulcergenic syndrome, described in 1055, consists of massive gastric hypersecretion, a fulminating ulcer diathesis with or without diarrhea and a non-beta islet cell tumor which has been shown to elaborate the hormone gastrin. More recently, the diarrheogenic syndrome has been reported. This disease entity—consisting of watery diarrhea, hypokalemia and achlorhydira—is felt to result from islet cell tumor production of the hormonesecretin.More than 50 years after their original descriptions, the first two gastrointestinal hormones, secretin and gastrin, have been implicated in clinical syndromes caused by non-beta islet cell tumors of the pancreas.
