Sodium homeostasis is maintained primarily by the balance of intake with the urinary excretion of sodium. The regulation of the rate of sodium excretion is dependent upon complex control systems which involve several levels of functional organization. These levels include the interaction of the kidney with other organ systems, such us the liver and the central nervous system; control systems which depend upon the kidney itself as a functional unit; the individual nephron level of organization; and finally the cellular regulators of sodium reabsorption. The modulation of sodium excretion involves the integrated function of all of these levels of organization. Many questions concerning regulation of sodium excretion remain open. Of primary interest are the possible existence of undefined hormonal systems and the means by which the various components of the system are integrated to achieve smooth regulation of sodium balance.

Daily intragastric administration of 50 or 25 mg/kg of cephalexin, oxacillin, cloxacillin, dieloxacillin and nafeillin to monkeys beginning eight hours post-intravenous challenge with a beln hemolytic streptococcus was followed by mortalities of 1 of 8, 2 of 8, 5 of 8, 2 of 8, and 5 of 8, respectively, whereas all eight controls died. Duration of acute illness and time required for apparently total clinical recovery were significantly shorter in survivors given cephalexin than in those receiving cloxneillin or nafeillin. Blood cultures were negative by the end of the len-day therapy period in seven, three and six survivors given cephalexin, eloxaeillin and dieloxacillin, respectively, whereas cultures were still positive in 2 of 6 and 2 of 3 receiving osacillin and nafeillin, respectively. Bacteriolngie, but not clinical, relapse was observed after therapy was discon-continued in 3 of 7, 2 of 6, 1 of 3, 4 of 6 and 1 of 3 monkeys treated with cephalexin, oxaeillin, ctoxacillin, dicloxacillin and nafeillin, respectively. In normal monkeys serum antibacterial activity after administration of isoxazoly penicillins followed the pattern ohserved in normal humans, but in infected monkeys this relationship was not consistently observed. Nafeillin also showed erratic absorption. Sera from cephalexin-treated monkeys showed more uniform autislreptococcal activity from day to day than did sera from monkeys given the four penicillins. These data add further evidence that cephalexin shows promise as an effective agent against both penicillin-resistant Maphylococci and sensitive cocci such as the beta hemolylic streptococcus.

Potassium depletion has been associated with glucose intolerance in primary and secondary aldosterouism, in uremia, and after the administration of kaliuretie drugs. The common occurrence of both potassium depletion and diabetes in Laenuec'x cirrhosis' suggested a causal relationship. Serum glucose and insulin responses to oral and intravenous glucose tolerance tests were measured in ten stable eirrhotic patients before and after the administration of 120 to 180 mEg KCl daily for at least ten days. Oral glucose tolerance, which was abnormal in all ten patients before KCl became normal in five after KCl. Total insulin increments, which were greatly increased before potassium in these patients, were not significantly changed after KCl. The five patients whose glucose tolerance improved after KCl were characterized before KCl administration by mild impairment of glucose tolerance and by the presence of an immediate, vigorous insulin response to intravenous glucose, findings compatible with an early abnormality of carbohydrate metabolism. The patients who failed to improve after KCl had had more severely impaired glucose tolerance and a diminished or absent prompt insulin response to intravenous glucose, abnormalities suggestive of more advanced impairment of carbohydrate metabolism. Although the mechanism of the potassium-associated improvement is not known, it was not associated with an increased insulin response, the expected consequence of potassium repletion.

To determine whether small oral doses of reserpine would have cardiovascular and cntecholamine-depleting effects on normal young men, six volunteers were given 0.25 mg b.i.d. of the drug for three weeks. Daily blood pressure and radial pulse rates were obtained in the supine and erect positions and 24-hour urines were analyzed for vanilmandelic acid (YMA). This metabolite was significantly depressed approximately 25% below control values after four days of reserpine and remained at this level throughout the remainder of the study. The orthostatic fall in systolic pressure was the most sensitive index of drug effect as it was significantly decreased after the second day. Supine systolic pressure became significantly lowered by the end of the first week and pulse rate changed a few days later. Alterations in the supine diastolic blood pressure were not discernible until the third week, whereas orthostatic diastolic changes were not influenced. It is concluded that these parameters can serve as useful indices of the effect of small oral doses of reserpine in normal subjects.

Two patients with autoimmune hemolytic anemia did not improve heparin therapy, though other measures did succeed subsequently. Nor did repealed Cr autologous red cell survival studies suggest that heparin altered hemolytic rates significantly. It is concluded that heparin therapy will not serve a useful role in all cases of autoimmune hemolytic anemia.

An instrument is described which provides instant numerical readout of the Achilles relax time after each tendon lap. This eliminates calculation errors, as well as the time involved in reading the multiple laps required in each patient. Immediate availability of a series of results allows application of standards of acceptability which were previously difficult to apply. A method of standardizing reported results is suggested which may help to reduce inter-observer and inter-group variability.
