Fourteen patients with bone and joint infections caused by a variety of bacterial pathogens were treated with intravenous cefamandole, 4to 12g/day; ten received additional therapy with 2 g/day of cefaclor and one with 2 g/day of cephalexin. Ten of 14 patients were cured. Of those cured, infecting pathogens had minimal inhibitory concentrations of cefamandole of 4 μg or less/ml. Of those who were not cured, two had recurrent infections that had not responded to other previously administered antibiotics, one had a mixed infection with resistant strains, and one had positive bone cultures five months after a favorable clinical response. Peak and valley cefamandole serum concentrations were 48.0 to 173.0 μ/ml and 23.4 /μg or less/ml, respectively. Synovial fluid concentrations were 11.1 to 16.7 μ/ml and bone concentrations were 0.2 to 20.4 μg/g. Adverse effects included leukopenia in one and hepatic enzyme elevations in four patients. Cefamandole, with and without cefaclor, was efficacious in therapy of bone and joint infections caused by susceptible organisms.

Variability in the levels of epinephrine (E) and norepinephrine (NE) in adrenal vein blood is thought to preclude their usefulness in caval sampling studies for localizing abdominal pheochromocytoma. In this study we demonstrate, in three patients, that the fraction of the total catecholamine level in the adrenal effluent represented by E(E fraction) may differentiate the normal adrenal medulla from the gland containing a pheochromocytoma. This observation is additionally supported by calculations of the E fractions, from adrenal venous E and NE reported recently, in three other patients with pheochromocytoma. Our data, combined with those from the literature, yield a mean E fraction, in adrenal vein blood, of 63.8 ± 11.4 (SD)% for seven normal adrenal medullae, and of 28.5 ± 4.8% for five glands bearing pheochromocytoma. The use of this parameter for lateralizing adrenal pheochromocytoma is suggested if other procedures are unable to demonstrate the tumor.

Rowachol, a mixture of 6 terpenes in olive oil and under investigation for dissolution of gallstones in humans, was compared with UDCA in hamsters with induced cholesterol gallstones. Eighty hamsters were allocated to eight groups of ten animals each. One group received only standard rodent chow. The other seven groups received the lithogenic regime (standard chow containing ethinyl estradiol and increased cholesterol), either alone or with 20 mg/kg/day of UDCA, or 5, 10, or 20 mg/kg/day of mixed terpenes in olive oil or 10 mg (of terpenes) kg/day of Rowachol or 0.2 cc/day of olive oil. The animals were sacrificed after 12 weeks. Two additional groups of six hamsters each received the lithogenic regime for 12 weeks, and then were switched to the standard diet, alone or with 10 mg/kg/day of Rowachol for eight weeks at the end of which time they were sacrificed. Rowachol decreased HMGCoA reductase activity 18%, but did not dissolve gallstones. Neither the terpenes nor Rowachol altered the biliary cholesterol saturation index, bile acid pool size or the activity of cholesterol 7-alpha hydroxylase or prevented formation of gallstones. UDCA unsaturated bile, increased the total bile acid pool size 38%, depressed the activity of HMGCoA reductase 29%, and prevented for mation of gallstones.

The efficacy of99mTc cholescintigraphy in the diagnosis of gallbladder disease was studied in 116 patients. Sixty-one demonstrated gallbladder visualization (normal) and 55 had nonvisualization. In patients with acute abdominal pain, test sensitivity is 100%, specificity is 77%, and accuracy is 83% for the diagnosis of acute cholecystitis; no patient with gallbladder visualization had acute cholecystitis. Test sensitivity for any form of gallbladder disease is 66%, specificity is 82%, and accuracy is 71%; excluding patients with laboratory evidence of hepatobiliary disease, the predictability of acute or chronic cholecystitis increases to 100%. Cholescintigraphy demonstrated common bile duct patency in all eight post-cholecystectomy patients referred for evaluation of possible choledocholithiasis. Thus, gallbladder visualization with99mTc-PIPIDA virtually excludes the diagnosis of acute cholecystitis, an abnormal test is a good predictor of gallbladder disease because of high specificity, and cholescintigraphy is extremely safe and simple to perform and may be used to demonstrate common bile duct patency.

Two cases of extreme granulocytosis are reported, emphasizing the clinical implications and differential diagnosis. Leukemoid reactions to malignancy, not chronic neutrophilic leukemia, will comprise the majority of such cases when the granulocytic response contains largely mature cells, whereas confusion with chronic granulocytic leukemia may result when peripheral immaturity is more manifest. Distinguishing features are discussed.

A case of scarlet fever studied in 1959 and caused bystaphylococcus aureus, phage type 52/52a/80 infection of a surgical burn is reported. The literature is reviewed and data are presented which indicate the distinct antigenicity of the erythrogenic toxins of staphylococci and group A streptococci. The patient developed neutralizing antibodies to staphylococcal toxin which disappeared ten months after infection. The similarity of the rashes and desquamation of the skin of several diseases caused by staphylococci indicate at least one common toxin.
