A case of severe lactic acidosis associated with phenformin treatment is described, and reports of similar cases are briefly reviewed. It appears that phenformin alone may cause lactic acidosis. Sodium bicarbonate must be given in massive doses in some patients. Hemodialysis plays an important adjunctive role by removing lactate and possibly phenformin and by preventing or relieving circulatory overload.

Thirty-six patients with infiltrative ophthalmopathy of Graves' disease were given 25 to 366 mc doses of131I as therapy with intent to ablate residual thyroid function. Total ablation was accomplished in only one patient, yet symptomatic resolution of chemosis, lacrimation, and infiltration occurred in 28 patients (77 per cent). There was lack of improvement in proptosis in 27 patients (75 per cent). Proptosis was well tolerated when the irritative syndrome of chemosis, lacrimation, and infiltration had been ameliorated. There was no progression of the exophthalmopathy or visual loss resulting in blindness in these patients after therapy was instituted. Pretibial myxedema was present in nine patients with post-treatment improvement in two patients and complete resolution in seven patients. There were no observed adverse effects of131I therapy other than post-treatment thyroiditis in one patient. Long acting thyroid stimulator assays before and after treatment did not yield consistent conclusions.

During a four-year period from 1968 to 1971, 152 episodes of Klebsiella bacteremia occurred in 137 cancer patients. Fifty per cent of the patients had neutrophil counts of less than 100/mm3at the onset of bacteremia and 36 per cent had a falling neutrophil count. The majority of the patients had previous antibiotic, antitumor, or adrenal corticosteroid therapy. Only 38 per cent of the patients recovered from their infection. Poor prognostic factors included inappropriate antibiotic therapy, adrenal corticosteroid therapy, persistent neutropenia, the presence of shock, and multiple sites of infection. Gentamicin, cephalothin, tetracycline polymyxin B, and kanamycin were the most active antibiotics againstKlebsiellaspp. Although an increasing number of isolates developed resistance to some of these antibiotics during the early years of this study, this trend appears to be reversing.

Concentration of four immunoglobulins and orosomucoid, transferrin, ceruloplasmin, and α-2 macroglobulin were estimated in sera and synovial fluids in patients with rheumatoid arthritis and in degenerative joint disease. An increase in serum immunoglobulins in rheumatoid arthritis was found in 15 per cent for immunoglobulin G (IgG), 21 percent for IgA, and 6 per cent for IgM. Low IgA level was observed in 19 per cent of estimations. IgD was either normal or low. Although the level of immunoglobulins in rheumatoid synovial fluids was generally lower than in the corresponding sera, it exceeded that in the serum in 8 to 16 per cent. No relationships between the concentration of immunoglobulins to the age, sex, duration of the disease, sedimentation rate, titer of latex fixation, Lansbury index, or extent of the damage in two simultaneously aspirated joints were found. Comparison of the serum/synovial fluid ratios of various proteins has shown that in rheumatoid synovial fluids all proteins tested are present in higher concentration than in the synovial fluids in degenerative joint diseases. The ratios for IgG and to the lesser extent for IgM exceeded those theoretically calculated for their molecular weights. These observations imply both excessive exudation of plasma proteins to the synovial fluids and production in situ of immunoglobulins.

Pseudomonas cepaciais an organism which has been isolated with increasing frequency from hospitalized patients. The organism is resistant to most of the commonly available antimicrobial agents. A young boy who developed pneumonia due to this organism following cardiac surgery is described. The need for therapy with high doses of antibiotics in infections due to this organism is emphasized.

It is uncommon for adult patients with sickle cell anemia to have splenomegaly and quite rare for them to have calcification of the spleen. Two patients are reported, both of whom had splenic calcification and one of whom also had splenomegaly and calcification of the falx cerebri.
