摘要:
A 60-year-old man developed periodic alternating nystagmus in association with decreased vision due to cataracts. Prior to surgery, vision was limited to hand motion only in both eyes. An extracapsular cataract extraction with insertion of a posterior chamber intraocular lens was performed in the patient's left eye. On the first postoperative day, vision was 20/60 in the left eye and the nystagmus was absent with both eyes open. Periodic alternating nystagmus that occurs with poor vision is related to a loss of fixation. Surgery aimed at improving the visual status may be effective in extinguishing the nystagmus.

摘要:
The case of a 4$$-year-old boy with congenital vertical ocular motor apraxia who was otherwise developmentally and neurologically normal is reported. The presence of perinatal hypoxia in this patient may have been etiologic. While the presence of a supranuclear vertical ocular motor abnormality usually suggests a serious, acquired neurologic or systemic disease it may rarely occur as an isolated congenital finding, as demonstrated in this case.

摘要:
Skew deviation is a common neuroophthalmologic finding, the presence of which is classically thought to signify a brainstem lesion. It is frequently seen in conjunction with other posterior fossa abnormalities. We report the cases of five patients having skew deviations with raised intracranial pressure without posterior fossa structural lesions. Whereas skew deviation almost always does indicate intrinsic posterior fossa disease, our experience demonstrates that increased intracranial pressure by itself can cause a skew deviation, probably by causing a secondary brainstem dysfunction.

摘要:
A 26-year-old male drug abuser developed amaurosis fugax followed by retinal vasculitis in the right eye after nasal inhalation of methamphetamine. He gradually recovered, but required prolonged systemic corticosteroid therapy. We believe that the amaurosis fugax was due to drug-induced vasospasm and that the retinal vasculitis was a drug-induced hypersensitivity vasculitis.

摘要:
A recent case of childhood dermatomyositis with striking retinopathy is reported. The patient's vision was initially at the 20/80 to 20/100 level with associated macular edema and extensive cottonwool infarction of the retina. Visual recovery to the level of 20/20 in both eyes was achieved following treatment with high-dose corticosteroids.

Two patients with visual loss after cataract extraction were found to have prominent pallid optic disc edema, peripapillary hemorrhage, and cystoid maculopathy. A diagnosis of anterior ischemic optic neuropathy, with coincidental aphakic cystoid macular edema, was initially considered. The clinical course, however, favored a diagnosis of visual loss secondary to maculopathy. In both cases, fluorescein angiography disclosed leakage of fluorescein dye from macula, optic disc, and peripapillary foci. Fluorescein angiography and stereo color photographs of 51 patients with aphakic cystoid maculopathy were reviewed to determine the incidence of optic disc edema. Although optic disc swelling was rarely recognized on fundus photography, optic disc hyperfluorescence was observed in 75% of cases.

Five male patients sought medical attention for diplopia and were found to have third nerve palsies secondary to pituitary adenomas. In four cases this was the only neurologic abnormality. In the fifth there was an additional, asymptomatic, temporal visual field defect in one eye. Partial third nerve involvement was seen in all but one patient. Four patients underwent successful surgery and were found to have chromophobe adenomas. One patient had clinical and laboratory evidence of Cushing's syndrome. An initial diagnosis of cerebral aneurysm was made in three cases when the third nerve palsy followed a severe, acute headache. Later it became evident that pituitary apoplexy was the correct diagnosis. The mechanisms by which a pituitary adenoma causes a third nerve palsy are discussed. The importance of recognizing a pituitary tumor as the etiology of an isolated third nerve palsy is emphasized.

摘要:
Progressive monocular visual loss was the presenting manifestation of a cystic ependymoma arising from the third ventricle. Extracerebral extension of the tumor into the prechiasmatic cistern compressed the right optic nerve. Subfrontal extension caused erosion of the planum sphenoidale. Subtotal excision of the tumor was followed by cranial irradiation, and no further visual loss occurred. Extracerebral growth of primary intracerebral gliomas may cause optic neuropathy that is indistinguishable from other compressive neuropathies.
