摘要:
A 40-year-old woman developed profound bilateral loss of vision from chronic atrophic papilledema. There were striking optociliary shunt vessels on the right optic disc. Within 1 month after removal of a massive intracranial meningioma, the shunt vessels had regressed markedly. This regression was evident on fluorescein angiograms. We attribute their resolution to the normalization of intracranial pressure.

摘要:
Optokinetic nystagmus was tested in patients with intracerebral tumors restricted to one hemisphere, by using a simple, hand-held drum and visual evaluation of symmetry of horizontal response. Computed tomographic mapping revealed that lesions associated with asymmetric optokinetic nystagmus involved the posterior hemisphere. The critical region appeared to extend from the posterior splenium and the anteromedial occipital lobe to the posterior internal capsule. It seemed to course below the posterior horn of the ventricular system, then turn anteriorly, and finally medially, above the temporal horn. It is argued that the posterior extremity of this C-shaped region contains the ipsilateral and contralateral visual inputs to a parietal cortex control over ipsilateral smooth eve movements, and that the anterior extremity contains the efferent motor command pathways destined to the brain stem.

摘要:
The clinical spectrum of electrical injuries is complex and diverse. Despite the high annual incidence of electrical accidents, little has been reported recently regarding the neurological deficits associated with this form of trauma. The most frequently cited reviews are those of Critchley (1), Langworthy (2), Naville and De Morsier (3), and Silversides (4). These authors have divided the neurological complications into three groups based on the temporal relationship between the shock and the manifestation of clinical signs: immediate, secondary, and delayed. The present case developed a dense homonymous hemianopia 4 days following an electrical accident. The details of the abovementioned classification, as well as a review of the pathophysiology, will be presented.

摘要:
Cecocentral scotomas are a hallmark of toxic, metabolic, and hereditary optic neuropathies, but are rarely associated with compressive processes. Acute visual loss due to a compressive optic neuropathy by a benign tumor is unusual unless intratumoral hemorrhage or infarction occurs. A case of acute, severe, remitting, symmetrical visual loss with cecocentral scotomas, initially thought to be of toxic etiology, proved to be due to a typical suprasellar meningioma originating in the olfactory groove. A history of olfactory disturbance with visual loss should prompt neuroradiologic investigation despite atypical neuro-ophthalmic features.

Paredrine has been used as an adjunct to the cocaine test for Horner's syndrome; however, there are no controlled studies measuring pupillary responses in normal eyes. Twenty-five volunteers with normal eye examinations received 1$$ Paredrine in one eye, followed by serial photography to document the extent of pupillary dilation. Pupillary measurements were taken from the photographs to prevent identification of the patient, control or treated eye, or the time after instillation of the eye drops. Lighter irides dilated more rapidly, and some patients with dark irides showed little response. The control eye showed progressive pupillary constriction as its fellow eve became more dilated.

摘要:
Edge-light pupil cycle time has been introduced recently as a simple method for examining parasympathetic pupillary innervation. Since the sympathetic system is not thought to be involved in the light reflex, it was assumed that edge-light pupil cycle time would be unimpaired in lesions of the sympathetic pathways.In 12 patients with unilateral Horner's syndrome of different etiologies, the edge-light pupil cycle time was determined in both eyes. In all cases the edge-light pupil cycle time was significantly prolonged on the abnormal side. Patients with central, preganglionic, or postganglionic lesions showed the differences consistently. These data support the notion that the sympathetic system plays a role in the pupillo-dilating phase of edge-light pupil cycle time, presumably by exerting a tonic mydriatic effect. Since this tonic effect is lost in Horner's syndrome, leading to slowed redilatation, the edge-light pupil cycle time becomes prolonged. Thus, edge-light pupil cycle time may prove to be a valuable diagnostic test for Horner's syndrome.

摘要:
The neuropathologic findings in an idiopathic case of the opsoclonus/myoclonus syndrome are reported. Although neurologic dysfunction may have been more widespread, structural lesions were limited to the cerebellum and inferior olives. Severe depletion of Purkinje cells with preservation of granular cells was evident throughout the neo- and paleocerebellum; however, groups of Purkinje cells were preserved in the archicerebellum. No abnormalities were evident in the paramedian pontine reticular formation of the caudal pons. Inflammation and evidence of anoxic damage were absent. These changes are very similar to those described in paraneoplastic cerebellar cortical degeneration.
