The case of a 29-year-old former parenteral drug abuser who presented with a 3rd nerve palsy and contralateral ataxia is reported. The patient was found to have a positive HLTV-III titer and acquired immune deficiency syndrome was diagnosed. Computed tomography demonstrated two ring-enhancing lesions in the brain which were presumed to be due to Toxoplasma gondii.

Spasm of the near reflex is most often seen on a functional basis in young adults with underlying emotional problems. In particular, when convergence spasm is associated with miosis on attempted lateral gaze, a functional basis for the disorder should be suspected. Patients who experience spasm of the near reflex following trauma commonly follow a benign course with spontaneous resolution of their ocular complaints within 1–2 years. Accommodative spasm, manifested by pseudomyopia, or spasm of convergence, alone, or in combination with miosis, may be found as isolated signs of spasm of the near reflex. We report a patient who continues to demonstrate accommodative spasm 9 years after a motor vehicle accident.

A 76-year-old man developed a cavernous sinus syndrome as the initial manifestation of multiple myeloma. Although clinically the patient had stage IA disease, which is typically associated with a favorable response to therapy, his disease was rapidly fatal. This case emphasizes a weakness of the traditional staging system because it does not take into account certain clinical and histopathologic aspects of myeloma relevant to extramedullary plasmacytomas. Since disorders of ocular motility are more likely to be associated with extramedullary myeloma than myeloma confined to the marrow, clinicians need to be aware of the limitations in the clinical staging system and the potential problems associated with anaplastic plasmacytomas.

We describe a patient with pituitary apoplexy causing sudden visual loss and light-near dissociation of the pupils but minimal ophthalmoplegia. Good visual recovery occurred despite an 8-day delay in neurosurgical treatment.

Four of 150 patients with myasthenia gravis had upper lid retraction. Only two had apparent ptosis in the contralateral eye. Three patients developed this paradoxical sign during medical treatment and after thymectomy. Hering's law of equal innervation does not adequately explain this phenomenon, and the possibility of cholinergic overstimulation in neuromuscular transmission is raised.

An 8-year-old boy with insulin-dependent diabetes mellitus and a seizure disorder demonstrated transient visual loss after severe seizure activity. The role of hypoglycemia in relation to his transient cortical blindness remains indeterminate. The nature of the cortical involvement, the rate of visual recovery, and prior reports of postictal phenomena emphasize the relatively benign nature of this condition in children.

Acute left homonymous hemianopia and recurrent scintillating scotoma occurred in a 43-year-old woman due to metastases from a cutaneous malignant melanoma that had been resected 5 years previously. Computed tomography initially demonstrated small, probably embolic, areas of occipital lobe infarction. Five months after the onset of her visual symptoms, massive cerebral hemorrhage occurred from a large right occipital lobe tumor that had not been present initially. Autopsy demonstrated malignant melanoma. Visual symptoms related to tumor embolization and/or cerebral hemorrhage may be the presenting features of metastatic cutaneous malignant melanoma.
