We document the case of a young woman with bilateral nonluetic, interstitial keratin's, vestibuloauditory problems, and bilateral recurrent facial palsies, who had strongly positive serologic tests of Lyme borreliosis. To our knowledge, this is the first reported case of positive serologic tests forBorrelia burgdorferiin a patient with Cogan's syndrome.

We present a patient with unilateral progressive painless loss of vision leading to optic atrophy and blindness. High-resolution computed tomography and magnetic resonance imaging demonstrated adipose tissue extending from the anterior portion of an enlarged optic canal posteriorly to involve the ipsilateral optic chiasm. To our knowledge, this is the first reported case of an intra-canalicular lipoma.

We report a patient with neuroretinitis, whose associated lymphadenopathy, exposure to cats, and strongly positive cat scratch antigen skin tests suggested the diagnosis of cat scratch disease. Cat scratch disease should be added to the list of infectious agents believed to produce the clinical picture of neuroretinitis.

We report a 10-year-old white girl who developed a pinealoblastoma 2 years before presenting to us with metastatic spread to the left optic nerve. She was treated for the original tumor with irradiation and chemotherapy. She presented to us with decreased vision in the left eye, unilateral disk edema, and diffuse left optic nerve swelling without increased intracranial pressure. Biopsy of the posterior optic nerve sheath was required for diagnosis. Vision was lost in the left eye, but the optic nerve swelling diminished after chemotherapy and additional radiation therapy.

A one-eyed patient was referred as a case of amblyopia secondary to myelinated retinal nerve fibers. Investigations revealed a pituitary adenoma. Following surgical ablation of the rumor, reduction in the myelination became evident. We report this rare occurrence in a case of pituitary tumor with photographic documentation for the first time.

We describe the visual illusions experienced by five patients with a history of previous use of hallucinogens, marijuana, or both. Symptoms included shimmering of images, illusory movement of images, visual persevera- tion of stationary objects, streaking of moving objects, and moving objects appearing as a consecutive series of stationary images. In all cases, the symptoms had persisted or recurred after periods of drug abstinence ranging from several months to several years. Despite thorough and repeated examinations and investigations, there was no evidence of neurologic ophthalmologic disease in these patients. When patients present with these and other visual illusions, a thorough drug history may afford the answer, provided that other recognized causes of these visual symptoms, such as migraine, epilepsy, and intracranial lesions have been excluded.

Summary: A 26-year-old woman sustained closed head trauma following a motor vehicle accident. Her injuries included complete trigeminal and abducens nerve palsies. Eleven months later eye muscle surgery succeeded in making her orthophoric in primary gaze although her right abduction deficit persisted. Eighteen months after injury the patient was able to fully abduct her right eye with jaw thrust to the left. With abduction of the right eye there was associated globe retraction and narrowing of the palpebral fissure. This is the second reported case of trigemino-abducens synkinesis. Mechanisms to explain this phenomenon are discussed.

Lateral rectus palsy is an uncommon event following contrast myelography. We report such a case, which fortunately proved to be transient, and speculate on its aetiology in terms of the anatomy of the sixth cranial nerve and the possible toxic effects of the contrast agent lopamidol (Isovue; ER Squibb and Sons, Princeton, New Jersey, USA; Niopam, E Merk, U.K.).

It has been stated that the inferonasal quadrant of the visual field is constricted in patients with dissociated vertical deviation. If this were true, it would be of clinical importance when assessing the visual field of patients with infantile esotropia. In addition, it could corroborate the hypothesis of an abnormal chiasmatic decussation in this condition. To assess this issue, we compared the sensitivity in the two inferior quadrants of the visual field in two groups of subjects by means of automated perimetry. The first group included 18 patients with infantile esotropia and the second, 14 normal subjects. Findings did not show any significant difference between these two groups of subjects and therefore indicate that changes in the inferonasal quadrant of the visual field are unlikely to occur as a result of infantile esotropia. The hypothesis of abnormal chiasmatic decussation in infantile esotropia is reconsidered, based on these results.

Lid lag in Guillain-Barre syndrome is not a well-known feature of the disease. Here we present two cases of Guillain-Barre syndrome with lid lag. In its formation, supranuclear levator dysfunction was the possible cause.
