A 39-year-old black woman presented with a clinical picture of aseptic meningitis. She was subsequently found to have Lyme disease, a tick transmitted spirochetal disease not unlike syphilis. Approximately 1 month after intensive antibiotic therapy including 128 million units of intravenous penicillin and 28 g of oral tetracycline, she developed symptoms of classical optic neuritis with painful loss of vision and optic disc edema. A discussion of this patient's clinical course, as well as the dermatologic, neurologic, and ocular manifestations of Lyme disease are presented.

Two patients experienced unformed visual hallucinations in one hemifield but only with vigorous exercise. The hallucinations continued for years as an isolated symptom before an occipital tumor was demonstrated. Exercise-induced unformed visual hallucinations appear to be ictal phenomena and might be a specific symptom of occipital brain tumors.

We report two patients who presented with intracranial hypertension without localizing signs and normal computed tomography (CT) scans. In both cases, the diagnosis of pseudotumor cerebri was erroneously made until the results of cerebrospinal fluid (CSF) analysis demonstrated cysticercosis. Our findings reinforce the concept that the diagnosis of pseudotumor cerebri should not be made without study of CSF, and add another cause for the syndrome of pseudopseudotumor.

Four patients with idiopathic pseudotumor cerebri were treated with Diamox, 1000 mg/day orally in divided dosages. Photographically documented resolution of papilledema was noted to begin after an average treatment period of 40 days and was complete in three patients by 91 days. The efficacy of Diamox in the treatment of papilledema resulting from pseudotumor cerebri has not been previously documented by fundus photographs.

Craniopharyngiomas generally are not considered in the differential diagnosis of pituitary apoplexy. A case of craniopharyngioma is presented, which emphasizes the need for magnetic resonance imaging in the setting of overt or subclinical pituitary apoplexy.

Bilateral optic nerve swelling occurred in a patient who had other findings typical of amiodarone toxicity. These included verticillate keratopathy, tremor, gait ataxia, and pulmonary fibrosis. Cessation of amiodarone therapy was followed by slow resolution of optic nerve swelling over 6 months; however, generalized constriction of the visual fields persisted. Several proposed mechanisms of optic nerve swelling secondary to amiodarone toxicity are discussed. Corroborative evidence in the literature suggests the possibility of a unique type of toxic optic neuropathy in this patient.

Cystinosis is an autosomal recessive metabolic disorder in which nonprotein cystine accumulates within most body tissues due to a defect in lysosomal cystine transport. The pathognomonic manifestation of cystinosis is the presence of distinctive iridescent crystals within ocular tissues. Although these crystals have been detected within the extraocular muscles, no associated abnormality of eye movement has been described. We measured horizontal saccades of 5–30|Mo in six patients with infantile nephropathic cystinosis. Our data indicate that patients with cystinosis have slowed saccades. Saccadic duration, peak velocity, peak acceleration, and peak deceleration were all abnormal.
