In order to lower the perinatal mortality rate, it is necessary to acquire a specific fetal diagnosis in all late abortions and stillbirths. A uniform classification for the maternal/fetal diagnosis is needed for comparison of progress in maternity care program between the different countries. We have studied 86 consecutive cases of late abortion and stillbirth at Viborg Hospital, Denmark, and made a comparison with the modified Aberdeen classification of fetal death.

The influence of episiotomy was evaluated in a quasi‐randomized follow‐up study. In 193 primiparas with spontaneous vaginal delivery, postpartum perineal pain and insufficiency symptoms and their duration were evaluated by means of a questionaire, 15–24 months post partum. Information about the birth and any complications was obtained from the medical records. On compairing the 85 primiparas delivered by midwives who had a low episiotomy rate (mean 21%) versus the 61 primiparas delivered by midwives with a medium episiotomy rate (mean 34%) and the 41 delivered by midwives with a high episiotomy rate (mean 70%), no differences were found regarding postpartum perineal pain and insufficiency symptoms. Intravenous infusion of buphenine hydrochloride was admistered on 4 successive days to 8 pregnant women with imminent preterm labor. Serial blood samples taken before and throughout the study were assayed for lymphocyte β2‐adrenoceptor density and cyclic adenosine‐3′,5′‐monophosphate (cAMP). The lymphocyte β2‐adrenoceptor density declined significantly (p<0.01) during the treatment. The plasma cAMP concentration was highest 4 h after commencement of infusion and decreased thereafter. Laparoscopic retrieval of oocytes from stimulated follicles for in vitro fertilization and embryo transfer (IVF‐ET) was performed at the time of diagnostic laparoscopy in 36 infertile women. Their mean rates of cleavage, embryo transfer and pregnancy did not differ from those obtained in 86 patients (101 cycles) who underwent ultrasonically guided transvaginal oocyte retrieval following diagnostic laparoscopy performed earlier as a separate procedure. In view of the advantages in terms of reduced patient morbidity, time and cost benefits, and the opportunity for oocyte‐sperm compatibility testing in vitro, the combined procedure appears to be the best method in patients scheduled to undergo diagnostic laparosc

It is generally believed that some human miscarriages result from embryotoxic factors existing in the sera. To study the embryotoxicity of such sera, 3.5‐day‐old mouse blastocysts were cultured for 72 h on 80% sera from different groups of women. After 72 h there was no blastocystic development in 53.2% of the cases grown on sera from women after two or more miscarriages, and none in 33.6% of the blastocysts grown on sera from women after one miscarriage, as compared with 8.2% and 12% respectively on control sera. Sera from women with miscarriages were divided into ‘high risk’ (50% or more embryotoxicity) and ‘low risk’ (less than 50% embryotoxicity) sera. The ‘high risk’ sera from two or more miscarriages caused an average of 72.1% undevelopment, while the ‘low risk’ sera (less than 50% embryotoxicity) from the same group caused 33.6% undevelopment. The ‘high risk’ sera from one miscarriage were embryotoxic to 55.8% of the blastocysts and the ‘low risk’ sera from the same group caused only 8.7% undevelopment. Serum concentrations of total cholesterol, high‐density lipoprotein cholesterol (HDL‐C), apolipoprotein A1 (Apo A1), apolipoprotein 6 (Apo B), and triglyceride (TG) were measured and that of low density lipoprotein cholesterol (LDL‐C) calculated in blood samples obtained from mentally handicapped women undergoing therapeutic amenorrhea (TA) induced by 5–10 mg of peroral lynestrenol, some receiving, some not receiving simultaneous anticonvulsant therapy Iphenytoin, carbamazepine or barbiturate, alone or in combination), In addition, these analyses were carried out in women receiving only anti‐convulsants and in controls (mentally handicapped women not receiving any of the above‐mentioned medications).Significantly lower HDL‐C, Apo A1, TG and cholesterol concentrations were measured in TA patients receiving lynestrenol only, than in those receiving anticonvulsants only, or in controls (p<0.001). With regard to HDL‐C and Apo A1, patients receiving both lynestrenol and anticonvulsants were intermediate between lynestrenol only patients and controls, but the HDL‐C/LDL‐C and Apo A1/Apo B ratios were similar to those observed in lynestrenol only patients.Addition of 8 or 12 mg of estriol succinate to the lynestrenol regimen was virtually without an effect. However, halving of the lynestrenol dose resulted in a significant increase in HDL‐C and in the HDL‐C/LDL‐C and Apo A1/Apo B ratios (p<0.001 or p<0.01), respectively. In 13 pregnant women with renal transplants the serum concentration of placental hormones were determined in order to show their ability to predict fetal problems especially placental insufficiency. It is shown that the hPL values were increased because of reduced elimination of hPL, presumably in the renal tubuli. In this study the weight of only one newborn was over the 50th percentile in spite of the fact that all but one had hPL values over the 50th percentile and in even 8 pregnancies the values were over the 95th percentile. In most of the growth‐retarded fetus's the ratio between hPL and the weight of the newborns was more complex.It is concluded that in renal transplantated pregnant women, hPL as a parameter for the placental function must be used with reservation and is not able to predict placental insufficienc

Of 91 women who underwent Burch colposuspension, 86 were available for clinical, urodynamic and isotope renographic follow‐up examinations 5 years after surgery for evaluation of late complications and long‐term effectiveness in urinary stress incontinence. A repeat urodynamic examination was accepted by 76 patients. Stress incontinence was cured in 71 % of the patients with a stable bladder preoperatively, and in 57% with stress incontinence and detrusor instability (non‐significant difference). Cure rate was not significantly related to age, hormonal status or previous anterior vaginal repair. The urodynamic measurements at follow‐up showed a significant increase in the functional urethral length in the cured group, as compared with the improved group. The mean maximum urethral closure pressure was reduced in both groups. Symptomatic detrusor instability was found in 18% of the preoperative stable bladders, while 67% of the unstable bladders had become stable. Only 29% of the patients with a preoperative unstable bladder had a normal lower urinary tract function at follow‐up. Late voiding difficulties were observed in 3% of the study group; enterocele, requiring surgical repair, developed in 7%. One patient suffered a damaged kidney due to undetected ureteral obstruction after surgery. Irritative bladder symptoms such as urgency, frequency, stranguria and nocturia represent a long‐term problem after colposuspension. At the 5‐year follow‐up, only 52% of the study group were completely dry and free of complications, and about 30% needed further incontinence therapy.
