Plasma testosterone, unbound testosterone, Sex Hormone Binding Globulin (SHBG) and albumin were studied in women with “non‐endocrine” ovarian carcinoma prior to and during chemotherapy. Fifty‐one postmenopausal or oophorectomized women with cancer of the ovary were studied. the histologic types were IC, IIC, IIIC, V and FIGO stages I–IV. Tumor volumes were evaluated once a month using bimanual recto‐vaginal palpation under anesthesia.Blood samples were drawn for testosterone radio‐immunoassay, SHBG and albumin analysis on four occasions at monthly intervals.Plasma levels were compared with a control group of postmenopausal women, a control group of fertile women in the follicular phase of the menstrual cycle, and finally a control group of postmenopausal women with non‐gynecologic disseminated malignant disease.Testosterone concentrations were found to be higher in women with carcinoma of the ovary than in postmenopausal controls and showed a relationship to tumor volume. Histologic type and FIGO stage were found to be less closely related to plasma testosterone concentration. No significant change was found in the unbound testosterone fraction. SHBG concentrations were elevated in the Large‐tumor group. A laparoscopic method of managing macroscopically benign, unilateral, unilocular, mobile, translucent, and smooth‐walled ovarian cysts in women of reproductive age is described. the method requires the cutting of a window in the cyst wall through the laparoscope fenestration. in preference to aspiration at laparoscopy or removal at laparotomy, this method has the advantage of ensuring histologic material as well as permanent drainage by the use of atraumatic surgical technique. Nineteen cysts were fenestrated laparoscopically. Four cysts could not be classified, two owing to lack of histologic material, two owing to malpreparation of the selected cyst tissue. in one case an epithelial cyst—a mucinous cystadenoma, being laparoscopically unsuspected—was fenestrated. The effect of dehydroepiandrosterone (DHA) on the lysyl oxidase activity in the mouse cervix was studied. Injection of animals ovariectomized 10 days before with 1 mg DHA was followed 24 h later by an increase in enzyme activity, which remained unaltered for 48 h, returning to control values by 96 h after injection. At least 100 μg of DHA was required to increase enzyme activity; the maximum effect occurred with 1 mg of the steroid and was comparable to that of 1 μg estradiol‐17β. Changes in 24‐hour excretion of unconjugated urinary cortisol and creatinine clearance were determined in 9 normally menstruating women and in 9 women undergoing estrogen–gestagen treatment on 3 consecutive days. A correlation between the cortisol excretion and the creatinine clearance was found only in the luteal phase of the menstrual cycle. the cortisol excretion increased neither during the menstrual cycle nor during the estrogen–gestagen treatment. In the present report we describe a new technique for monitoring intra‐uterine pressures during labor. the method utilizes a fiberoptic pressure transducer which is both accurate and stable. the pressure sensor is located at the catheter tip thus eliminating hydrostatic errors during monitoring. At body temperature the catheter is soft and flexible and represents no harm to the fetus. the membrane is housed in a metal dome with large lateral apertures. This arrangement greatly reduces the possibility of blockage. A comparison was made between recordings from the fiberoptic pressure transducer and those from a fluid‐filled catheter implanted simultaneously in non‐selected subjects during labor. Close agreement was obtained in the majority of cases. Large discrepancies between the two techniques were assumed to reflect the fact that the precise measuring point is not identical in each system. to date, recordings have been made in 52 parturients with no problems with insertion of the device and without any indication of equipment malfun

Following kidney transplantation, 20 women gave birth to 24 infants after the 28th gestational week. All babies were singletons, alive, and free from malformations. the mean weight was 2 595 g (range 1 420–3 200 g) and the mean gestational age was 37.8 weeks (range 32–40 weeks). the cesarean section rate was 75%. On dividing the patients into low‐ and high‐risk groups, the rate of pre‐eclampsia, prematurity, and intra‐uterine growth retardation was 2–3 times as high in the high‐risk as in the low‐risk group. No patients experienced graft rejection during the pregnancy but, within 3 months after delivery, two rejection episodes occurred. Later, a further 5 patients experienced graft rejection. All infants developed normally. We conclude that pregnancy following renal transplantation generally has a normal outcome and that the function of the transplanted kidney is unaffected

In order to obtain reference values during normal pregnancy 24 women with strict criteria for health and normal pregnancy were studied. Greatest interest was focused on blood pressure (BP) measurements and renal function tests. Investigations were made in early second trimester and in the 30th, 33rd and 36th gestational weeks. Gestational age was estimated by ultrasound measurement of crown‐rump length before the 14th gestational week. Both systolic and diastolic BP measured in the right arm were about 10 mmHg lower in the left lateral position than when supine or standing. the difference is suggested to be dependent on hydrostatic factors. Diastolic BP should be defined at the fourth phase of the Korotkoff sounds in order to be reliable because of the common phenomenon of late or non‐disappearance of the sounds during pregnancy. Diastolic BP in phase IV increased in up to 25% of the cases with 15 mmHg or more from early second trimester to the 36th gestational week in all three positions. Serum creatinine concentration was low in early second trimester and did not change during pregnancy, while serum urea decreased and serum urate increased during pregnancy. the results emphasize the importance of using reference values from a normal pregnant population obtained at different gestational weeks for comparison in studies on certain pathological conditions during pregnancy, especially pre‐ecla

Acetylcholine (ACh) is localized in the syncytiotrophoblast layer of the human placental villous tissue. an attempt was made to correlate the ACh synthesis in different pathological placentas with the histopathology of the syncytiotrophoblast available in the literature. the ACh synthesis was estimated by “in vitro” incubation of the placental tissue. Full‐term (36–38 weeks) vaginally delivered pathological placentas and hydatid moles (28 weeks) were compared with normal placentas of the same age. the results suggested that: (1) ACh synthesis is normal in states with normal syncytiotrophoblast (e.g., healthy>42 week placenta, placenta praevia, twins, and hydramnios); (2) high ACh synthesis is correlated with hormonal and immunological changes (e.g., diabetes mellitus and Rh‐incompatibility); (3) low levels of ACh synthesis occur in states with moderate syncytial degeneration (e.g., nephrotic syndrome and essential hypertension); (4) very poor ACh synthesis occurs when syncytial degeneration is advanced (e.g., preeclampsia, eclampsia, intra‐uterine death of fetus, vesicles of hydatid mole and placental tissue infarcts); and (5) ACh synthesis is nil in material that is completely devoid of syncytiotrophoblast (e.g., placental tissue‐like material, which rarely appears in between the vesicles of hydatid moles). in essence, the degree of reduction in ACh synthesis seems to correlate with the state of the syncytiotrophoblast in various pathological conditions; and ACh synthesis is greately reduced during syncytial degeneration. By studying normal labors in 500 Chinese primigravidae, a nomogram has been constructed to show the normal progression of cervical dilatation starting from cervical os of 3 cm dilatation. Paralleled “alert” lines and “action” lines were derived from this nomogram.
