Clinical and pathological analysis of vulvar carcinoma in 119 Jamaican women, treated at the University Hospital of the West Indies, over the past 18 years stresses that the tumor was more frequently seen in women under the age of 50—contrary to the experience of authors from the Western metropolitan countries. The natural history of the tumor showed that in 51% of cases, vulvar carcinoma co‐existed or followed previous chronic vulvar granulomatous disease and showed a rapid tumor progression in these post‐granulomatous cases. From 1961—66 to 1973—78 in Uppsala county, Sweden the overall incidence rate of ectopic pregnancy rose from 0.31 per 100 notified pregnancies or 0.28 per 1000 women 15—44 years of age, to 1.00 and 0.96, respectively. During the period 1967–75 severe cervical dysplasia was diagnosed in 87 consecutive cases by means of colposcopically directed biopsies of the portio and by cervical curettage. Thirty‐eight of the patients were treated, 3–6 weeks after diagnostic biopsy, by conization (conization group) and subsequently followed with smears and colposcopy. The remaining 49 patients (the biopsy group) were followed up at regular intervals by colposcopically directed biopsies of the portio and cervical curettage. The follow‐up period for all 87 patients ranged from 9 to 106 months, mean 47.6 months. The groups were comparable with respect to the most important epidemiological factors.During the follow‐up period progression to carcinomain situand to microinvasive carcinoma was found in 57.1% of the biopsy group but in 0% of the conization group. Of the whole material, 69 patients had conization (the 38 patients of the conization group plus 31 of the biopsy group). In 3 cases (4.35%) the cone diagnosis was 1 grade more serious than the biopsy diagnosis. The mean transit time of progression from severe dysplasia to CIS/MIC was 12–15 months.It is concluded that severe dysplasia is a rapidly progressing state which can be safely diagnosed by colposcopically directed biopsies of the portio and cervical curettage and effectively treated by conization. In view of the fact that not less than 32.7% of the cases with severe dysplasia in the biopsy group regressed during follow‐up, we recommend that the diagnosis be established by portio biopsies and cervical curettage, twice with an interval of 3 months to restrict the number of superfluou

The 24‐hour Variation in Total and Unconjugated Serum Estradiol‐17β as Well as the daily variation in one‐hour serum concentration values of FSH and LH have been examined in 9 post‐menopausal women given 2 mg tablets of estradiol‐17β daily for 14 days. The tablets were manufactured according to a new dispersion method aiming at producing essentially smaller crystals, thereby enhancing the bioavailability of the estradiol‐17β. In comparison with other preparations, this one provides a more rapid absorption, reaching peak effect after only 1 or 2 hours and presenting a higher peak than has been found with any of the others. The elimination from serum combines a rapid initial phase followed by a very slow one. Thus serum levels of estradiol‐17β 24 hours after administration are significantly higher than the levels measured before administration of the first tablet. A prospective pilot investigation of 14 women subjected to sterilization via laparoscopy, using bipolar electrocoagulation of the Fallopian tubes, did not reveal any significant change in the levels of follicular stimulating hormone (FSH), luteinizing hormone (LH) or estradiol either before or 3 months after the operation. However, the investigation gave evidence of a slight decrease in estradiol levels in the first 3 months after sterilization.

Bacterial cultures were obtained from intrauterine devices (IUD) removed from 274 patients admitted to the Department of Obstetrics and Gynecology of the University Central Hospital of Helsinki because of suspected pelvic inflammatory disease (PID). meno‐metrorrhagia, pain or other conditions potentially caused by an IUD. A diagnosis of serious PID was made in 42 patients, mild PID in 119 patients, while 113 were found to have no infection. An association between positive cultures of β‐hemolytic streptococci andEscherichia coliand PID was found. Positive cultures ofNeisseria gonorrhoeaewere significantly correlated with PID. The risk of developing PID was significantly greater during the first month after insertion of the IUD than when more than 30 days had elapsed from insertion of the IUD. Ultrasound measurement of fetal body dimensions provides a non‐invasive and precise way of obtaining information of fetal size. We applied clinically two methods for fetal weight estimation described by Hansmann. The first method employed ultrasonic measurement of the fetal biparietal diameter (BPD) and the transverse diameter through the lower fetal thorax aperture (THQ). This method was applied to 383 fetuses within 4 days of delivery. Weight range of the birth weights was 680‐5070 g. SD of prediction errors was 264 g. The method overestimated (p<0.01) the fetuses weighing less than 3000 g and underestimated (p<0.001) the fetuses weighing above 4000 g. The results for the fetuses weighing between 3000 and 4000 g were adequate. The second method employed the BPD, THQ and the gestational age for fetal weight prediction. We applied this method to 174 fetuses and obtained SD of prediction errors of 259 g. The results were adequate in all weight groups except for the fetuses weighing over 4000 g; this group was underestimated (p<0.001). The reason for the inaccuracy in the high weight groups is discussed. In 116 patients, abortion was induced in midtrimester by intra‐amniotic administration of 20% hypertonic saline and 25 mg prostaglandin F2α. Within 60 hours 113 aborted (success rate 97.4%). Of these 68.1% had aborted within 24 hours and 92.9% within 48 hours. Thirteen patients who had not aborted within 24 or 48 hours were treated with oxytocin drip. The prevalence of individual premenstrual symptoms was studied in a randomly selected group of 217 women taking oral contraceptives (OC). The results were compared with corresponding data from 595 women not using the pill. The analyses included six different mental and somatic symptoms defined and graded in severity and related to age. It was found that OC had a beneficial effect on the premenstrual tension symptoms (PMT) in all age groups except in the youngest group (18 years), in which the tendency was rather the opposite. Within the total sample of women, users of OC were significantly less frequently absent from work. The present results are in agreement with those of most earlier studies, provided that age is taken into account.
