Abstract.In 206 pregnant women with unfavorable cervix, labor was induced by local application of 1.5 mg PGE2by means of a portio‐adapter. In 70.4% labor was successfully induced after a single “PG‐cap”. An additional 11.6% went into labor after a second or third cap. In 14.1%, a sufficient priming of the cervix was obtained and amniotomy was performed. 3.9% had to be considered failures. The overall incidence of operative deliveries was 17%. Apgar scores and cord gases were within normal ranges. Internal tocography revealed a physiologic pattern of contractions with slightly elevated maximal pressures but normal baseline values. With local application of PG by means of a portio‐adapter the preparation of a gel mixture can be avoided and the exact anatomic location of the PG‐containg solution allows a withdrawal in case of hyperstimulation. Abstract.Three cases with metastasized choriocarcinoma are reported. The basic chemotherapeutic regimen was methotrexate and actinomycin D, combined with cyclo‐phosphamide or vinblastine. Surgery was used in the treatment of both primary and metastatic lesions. The chief indications for hysterectomy arise when chemotherapy has obviously failed. Thoracotomy was performed for a pulmonary metastasis and nephrectomy for a metastasis of the kidney. Craniotomy was performed to control the brain hemorrhage. Abstract.Pregnancy‐specific β1‐glycoprotein (SP1) levels in uncomplicated single pregnancies were measured by radio‐immunoassay from the time of ovulation until the 8th week of pregnancy in 129 blood samples from 78 women. SP1was detectable in all samples examined from 34 days after the last menstrual period (LMP), and thereafter the SP1level increased rapidly with time and 38 to 40 days after LMP the geometric mean SP1concentration was 90 μg/l. From pregnancy weeks 8 to 41, SP1levels were measured by nephelo‐metry in 1 255 blood samples from 1 255 women. A 95% reference range was established using logarithmic transformation. There was a steady increase in the SP1concentration until the last month of pregnancy, in which a tendency to level off was seen, the geometric mean levels ranging from 149 to 170 mg/1. The day‐to‐day variation was studied during 5 consecutive days in 10 women. No significant variation was found. The diurnal variation was studied in blood samples taken every 4th hour during a 24‐hour period form 10 women. A significant decrease was found at midnight and at 4 a.m. The elimination rate of SP1from serum was studied in 10 women following labor. Elimination was non‐linear and the time taken for SP1to decrease to 50% ranged from 24 to 50 hours.No correlation was found between the concentration of SP1and parity, maternal age, or the sex of the infant. The SP1concentration was significantly lower in heavier women (>70 kg) than in lighter women (≤ 70 kg). Abstract.Pregnancy‐specific β1‐glycoprotein (SP1) levels were measured by nephelometry in sera from 107 women admitted to hospital because of vaginal bleeding in weeks 8 to 20 of pregnancy. The SP1results were compared with those in single samples from 655 women and serial samples from 9 women with uncomplicated single pregnancies in the same period. Abstract.Factors involved in blood‐oxygen transport were studied in 46 pregnant women during the first trimester. All had type 1 (insulin‐dependent) diabetes and comparisons were made with similar measurements from 19 non‐diabetic pregnant women, also in the first trimester. The concentration of hemoglobin1c(HbA1c) was significantly increased (7.6% versus 4.4%, p<0.01) and arterial oxygen saturation was decreased (0.95 versus 0.98 mol/mol, p<0.01) in the pregnant diabetics compared with the non‐diabetics. The hemoglobin concentration was significantly elevated in the diabetic women (12.9 versus 12.1 g/100ml, p<0.01). Even though the red cell 2,3‐diphosphoglycerate content was the same in the two groups, and pH was significantly lower in the diabetic women, hemoglobin‐oxygen affinity was slightly increased in the diabetic patients (P50at actual pH: 26.2 versus 26.7 mmHg, p<0.05; P50at pH 7.40: 27.0 versus 28.0 mmHg, p<0.01). The study has demonstrated certain modifications in the blood oxygen transport system in the first trimester of pregnancy of diabetic women that are possibly related to the presence of excess amounts of glycosylat‐ed hemoglobin with increased oxygen affinity. This disturbance in maternal oxygen transport, particularly when associated with diabetic vascular disease, may lead to episodes of fetal hypoxia. Abstract.Thirteen women aged 36—46 years (mean 43) who were seeking contraceptive advice, were monitored for 3 months with basal body temperature (BBT) and were then treated for 3 months with CyclabilR(11 tablets of 2 mg es‐tradiol valerate (E2) followed by 10 tablets of 2 mg E2+ 0.5 mg norgestrel). This treatment was immediately followed by another 3 months with CyclabilR+ 2 mg E2. BBT revealed a spontaneous ovulatory rate of 89%. Cyclabil reduced the ovulatory rate, as measured with repeated serum progesterone analyses, to 37%, and Cyclabil + 2 mg E2resulted in a marked reduction in the number of ovulations (3 %). Both types of hormonal combination resulted in relatively small changes in Sex Hormone Binding Globulin Capacity. It is concluded that the addition of 2 mg E2to CyclabilRcauses a decrease in ovulation rate to such an extent that women using this combination run a relatively small risk of conceiving. Abstract.Various seminal variables were studied in two groups of infertile couples and in one fertile control group. Sperm morphology was emphasized. Patients with normal postcoital tests and sperm counts exceeding 5 mill/ml were selected and followed prospectively. The two clinical groups could then be identifieda. couples who were persistently infertile during the 3 years of observation and whose infertility was not explained by the clinical investigation, and;b. couples where conception was achieved during the period of observation, either spontaneously or after treatment of the woman. In the latter group all the variables of semen analysis were identical with those of the control group. Differences between the two groups of infertile patients were noted only with respect to the morphology of live spermatozoa of the semen sample and of postcoital spermatozoa within the cervical secretion. It is concluded that these two variables of sperm evaluation are of prognostic significance with regard to future fertility.

Abstract.A prospective study of the clinical value of the symphysis‐fundus (SF) distance was performed on 528 women, all at risk for developing intrauterine growth retardation (IUGR). Four different types of SF curves were observed and denoted as normal, static, catch‐up and low. A normal or static SF curve predicted a normal birthweight (specificity 92%), while a catch‐up or low SF curve identified 79% of the infants with a birthweight below 2 SD for length of gestation.The differences in short‐term morbidity between infants delivered by mothers with normal and pathological SF curves were mainly due to prematurity.We find the SF method to be a useful tool for detection of IUGR.
