During the first twelve months of its existence, our RICU treated 140 patients in acute respiratory failure with a survival rate of 128/140 (91%). Fifty-four of these patients required mechanical ventilation for > 24 hr. The survival rate in the mechanically ventilated patients was 42/54 (78%). Eight of the 12 deaths were related to sepsis. The most frequent complications of respiratory failure were atelectasis and pneumonia. A unit excelling in respiratory care provides a sound foundation for multidisciplinary support of the critically ill.

Central and peripheral temperature patterns in rewarming cardiac surgery patients have been studied by thermistor and infrared scanning techniques. These studies were combined with comprehensive patient monitoring records in a series of five patients. Evidence that thermoregulation, cardiac output, and drug action strongly affect the rewarming patterns could be substantiated for this limited series.In general an anatomic sequence of rewarming occurs in which the toes rewarm before the entire foot. Patterns of rewarming at the fingers and hands were similar to those at the toes. There was interpatient variation in the detailed temporal-spatial re-warming pattern of the digits. Blood flow rates to the toe were computed in accordance with the temperature data and were substantiated by venous occlusion plethysmography.

The effect of doxapram hydrochloride on the cardiovascular system was observed in critically ill patients in whom doxapram was utilized as a pharmacologic stir-up regimen. Improved cardiac output was one of the most pronounced changes observed. The systemic arterial pressure also increased, but systemic vascular resistance remained essentially unchanged; this pressor response was greater in hypotensive patients than in normotensive ones. Increased cardiac work and insignificant changes in central venous pressure suggested improved cardiac function. Calculation of the degree of venous admixture indicated a concomitant decrease in pulmonary shunting. While oxygen consumption did increase significantly, there was a comparable increase in oxygen availability, presumably because of the improvement of flow and ventilation-perfusion relationships. We conclude that doxapram may be used safely as an emergency measure in the critically ill, provided that there is no impairment of cardiac functional reserves.

External counter pressure of 20 to 25 mm Hg with the application of a G-Suit was found highly effective in the control of massive retroperitoneal hemorrhage in two patients with severe pelvic fractures. In addition, the technique provided immobilization of the fractures and facilitated the overall care of the patients.

Symptoms of mesenteric arterial insufficiency often prompt surgical intervention which incurs high morbidity and significant mortality. In certain individuals in whom a relationship can be demonstrated between bradycardia and symptoms of arterial compromise, pacemaker insertion may increase cardiac output sufficiently to offer an alternative for patients who are poor operative candidates.

Ikeda designed a flexible fiberoptic bronchoscope in 1966 and subsequently reported its ad-vantages over the rigid bronchoscope in the diagnosis of bronchogenic carcinoma.1Since then, this instrument has gained widespread popularity and has seen a broad expansion in its applications. Serious trauma to the trachea (eg, tracheal stenosis, tracheomalacia,land tracheoesophageal fistula) can result from the use of cuffed tracheostomy tubes. Cuffs with flat sides made from high compliance rubber or synthetic material have reduced but not totally eliminated the incidence of tracheal damage. Therefore, periodic examination of the tracheal wall and the caliber of the trachea during prolonged mechanical ventilation and after extubation can assist in the detection of covert, overt, or latent trauma. Direct stomal tracheoscopy with a Wis-Hipple Blade (#1) is a safe and effective procedure for such examinations.
