AbstractThe U.S. Olympic Team demonstrated a significant prevalence of E.I.B. among world‐class athletes (15.1). Nearly all sports were involved but endurance events seemed to have the highest prevalence of team members with E.I.B. Resting pulmonary function studies were poor discriminators of athletes affected with E.I.B. The 67 athletes of the 597 member team were responsible for winning 41 Olympic medals including 15 golds and 21 silvers, clearly demonstrating their ability to compete and succeed in world‐class competitive events. AbstractIn this paper, we describe chemical mediator fluctuation in exercise‐induced asthma (EIA) and exercise‐induced late asthmatic responses (EILA).The moderate, severe patients who showed EIA, had statistically significant increased plasma histamine levels after exercise. Leukotriene (LT) release from polymorphonuclear leukocytes (PMN) showed two different patterns.In the EIA negative group, generation of LTs from PMN showed a significant increase after an exercise task. However, the EIA positive group showed statistically significant decreases at 15 minutes after exercise.In EILA, patients showed biphasic decreases in FEVi.o following exercise, which was accompanied by a parallel increase in neutrophil chemotactic activity (N

AbstractExercise is a powerful stimulus to the development of asthma. In most asthmatic subjects the airways obstruction recovers spontaneously within 60 minutes, but in some subjects there is more prolonged airflow obstruction which requires bronchodilator treatment. Approximately 40–50% of subjects with EIA will show a refractory period of 2–4 hours after an initial exercise task, during which time an identical exercise task will evoke significantly less (%lt;50%) bronchoconstriction. In some patients, particularly children, EIA will be followed 3 to 9 hours later by a further episode of bronchospasm, termed the late asthmatic response. AbstractIn the preceding studies, protoplast‐like “spherical bodies” were detected electron microscopically in mice which were challenged intramuscularly, approximately 15 days before, with an attenuated strain ofStreptococcus pyogenes. The “spherical bodies” were 0.5–1.5 Mm in diameter, poor in ultra‐structural preservation of the cytoplasm, devoid of cell walls, and indistinguishable from those discovered in the buffy coat of peripheral blood from patients with mucocutaneous lymph node syndrome (MCLS).The present investigation was intended to elucidate the intracellular localization of the “spherical bodies” and to understand better the mechanisms responsible for the elimination of viable streptococci from the infected sites. SummaryWe investigated methods for measurement of β2‐microglobulin, lysozyme, IgG and other globulin fragments such as Fc, F(ab')2,K‐and Δ‐chains in unconcentrated urine. A log‐log correlation was observed between the percent transferrin clearances and the molecular weights of β2‐microglobulin, lysozyme, transferrin and IgG in normal children by quantitative analysis. Glomerular damage was suggested in patients who excreted urinary protein in excess of 1 g/m2/ day. The selectivity patterns showed persistence of glomerular disorders in patients with membranoproliferative glomerulonephritis, crescentric glomerulonephritis, glomerulonephritis due to Henoch‐Schonlein purpura and lupus nephritis, in spite of decrease in urinary protein excretion after treatment.Scanty globulin fragments were found in urine. The ratio of dimer‐ and monomer‐light chains found varied depending on the pathogenesis.Electrophoretic figures seen by the Western blot method were able to differentiate urinary proteins using unconcentrated urine.A combination of the procedures in this report could be useful to evaluate the severity and to monitor the progr

AbstractPrealbumin is a so‐called rapid turnover protein which has a relatively short half‐life of two days. It has been shown to be a useful indicator of nutritional adequacy in premature babies. Retinol‐binding protein (RBP) and transferrin are two other rapid turnover proteins. The purpose of this study was to determine whether serum retinol‐binding protein and transferrin can be used for evaluating protein‐calorie adequacy in neonates, by correlating serial measurements of their concentrations with serum prealbumin concentrations. Prealbumin, transferrin and RBP concentrations were measured by a single radial immunodiffusion method. Our study showed that there is a significant difference in mean concentration of serum prealbumin between full‐term AFD (appro‐priate‐for‐dates) and SFD (small‐for‐dates) babies (p<0.05), but not in preterm babies. In normal birth‐weight infants, the concentration of RBP correlates positively with prealbumin (r=0.71), but this correlation was not observed in low birth‐weight infants (r = 0.58). We speculate that the concentration of RBP is an indicator of conditions other than protein nutrition. The serum concentration of transferrin does not correlate with prealbumin. AbstractMortality and morbidity of small‐for‐gestational‐age (SGA) infants with very low birth weight (VLBW) (less than 1,500 g at birth) are reported. Survival rate of 47 VLBW infants was 92%, with no statistical difference between SGA and appropriate‐for‐gestational‐age (AGA) groups. Thirty‐nine VLBW infants were watched in our follow‐up clinic for more than seven months. Overall incidence of major neurologic defects (cerebral palsy, epilepsy or psychomotor retardation) was 12.8%. Handicap was significantly different for SGA and AGA groups, that is, four out of 12 SGA infants (33%), compared with one out of 27 AGA infants (3.3%). Major neurologic defects in VLBW infants were strongly associated with intrauterine growth retardation (IUGR) related to toxemia of pregnancy with onset in the 2nd trimester. Although most of the VLBW SGA infants were IUGR due to noxious intrauterine environments, head sizes of all, with one exception, were growth‐restricted in utero.
