AbstractA negligible, if not entirely negative, production of antibody to Epstein‐Bart virus(EBV)‐associated antigens in patients with mucocutaneous lymph node syndrome(MCLS) has been a perplexing problem for investigators who are in favor of a streptococcal etiology of the disease.In the present investigations, antibody formation to EB viral capsid antigen (VCA) was assessed by the use of the indirect immunofluorescence technique in mice which had been subjected to neonatal infection with an attenuated strain ofStreprococcus pyogenesor inoculation with either heat‐killed streptococci or streptolysin‐O, both of which were emulsified in Freund's complete adjuvant (FCA).Each mouse of these groups was inoculated 30 days or so later with EBV dilution in FCA, blood was collected about three weeks after virus infection, and antibody titers were assessed individually.According to the results of these experiments, no apparent difference was detected among the mice, irrespective of the methods of their neonatal treatment concerning their refractoriness to EB‐VCA; the majority of the animals showed titers of less than 1:8, no case having a titer above 1:30. In contrast, all sera from control mice yielded positive responses showing antibody titers of 1:250 and over.Such an immunological character is an exact counterpart of that of MCLS patients. AbstractThe relationship between the deposition of fibrin/fibrinogen related antigen (FRA) and morphologic findings in glomeruli was investigated in 59 children with IgA nephropathy. After treatment of renal biopsy sections with monochloroacetic acid (MCA) solution, MCA‐insoluble FRA, suggesting cross‐linked fibrin, was observed within the capillary walls, the mesangium and/or the sclerotic area in children with severe proliferative lesions, such as segmental sclerosis, capsular adhesions or crescent formation. Mesangial deposits of FRA were confined to electron‐dense deposits, and the MCA‐soluble FRA did not seem to be closely related to glomerular damage. AbstractTo evaluate the hand bones of patients with juvenile rheumatoid arthritis (JRA) objectively, 30 patients with JRA were examined by microdensitometric analysis (MD). The results were as follows: 1) Among the nine indices of MD, %σGS/D, reflecting the bone mineral content, correlated well with the stage of JRA determined by roentgenological findings of the hand bones. 2) %σGS/D showed remarkable improvement after the remission of JRA. 3) Rheumatoid factor (RF) positive JRA showed statistically low %σGS/D compared with RF negative JRA, and kept a low %σGS/D for a long period from the onset in contrast to RF negative JRA. 4) %σGS/D in the patients with JRA treated with steroid decreased more rapidly according to the dosage of steroid than in the patients with SLE. Based on these results it is suggested that MD seems to be useful in evaluating the bone objectively, and not only steroid but also RF seems to be a factor leading to bone destruction in patients with

AbstractIn the two health centers of Sapporo City, 112 mothers whose children participated in a three‐year health examination, and 107 mothers whose children did not, were interviewed about their children's health.More time and expense were necessary for the latter group to come to the health centers. Some children did not attend because of apparent indifference of their mothers to health matters. In both groups there was a tendency for attendance for vaccinations and health examinations to decrease as the child's age advanced, especially after four months.For the purpose of increasing the number of children who receive a three‐year health examination, provision of appointments extending over several days with extra examinations in places outside the usual health centers, in addition to making available detailed information on the examination, seem to be effective.A considerable proportion of mothers accepted re‐screening for neuroblastoma, for which most could afford 1,00

AbstractIn order to evaluate a new enzymeimmunoassay (EIA) of thyrotropin(TSH) for neonatal hypothyroidism, 6,100 blood samples on fdter paper, obtained through a neonatal hypothyroid screening program, were examined using a one‐step sandwich EI A and a radioimmunoassay(RIA). Measured TSH values, numbers of cases and false‐positives detected, and recall rates, were compared between the two methods. During this study two cases of congenital hypothyroidism were found by both EIA and RIA. However, a patient with hyperthyrotropinemia was detected only by the EIA. The reproducibility of the EIA was slightly better than that of the RIA. These results suggest that the EIA has same or higher sensitivity for detecting patients compared with the RIA. On the other hand, three cases with falsepositive elevation of TSH by EIA were found. High TSH values by the EIA were likely to be due to transplacental factors, probably immunoglobulin G. Because of high reproducibility of the EIA and false‐positive cases found with it, recall rates of the screening program were slightly higher for the EIA than for the RIA. The recall rate of the EIA could have been decreased by setting a cut‐off point at a higher percentile. Since the EIA is simple to use, this method is suitable for screening for congenital hypothy

AbstractThe blood ammonium level was monitored in low birth weight infants (<2,500 g at birth, N=117). There was no significant correlation of the blood ammonium level with gestational age, body weight at birth or the Apgar score. When the blood ammonium level was compared in infants fed with sufficient amounts of arginine‐supplemented (N=29) and non‐supplemented formulas (N=12) in addition to the mothers' milk, the blood ammonium level was found to be significantly lower in the former group than in the latter group (50.1 ± 12.4 μmol/L vs 40.0±12.2 μmol/L; p<0.02). AbstractThe clinical findings in a female infant with a terminal deletion (1) (pter→q42.3:) are described and compared with 18 other cases displaying a similar chromosomal abnormality. bed on an analysis of the 19 cases, postnatal growth retardation, microcephalus, severe mental and motor retardation, muscle hypotonia, high‐pitched shrill cry, and hypoplasia of the external genitalia in males were almost invariably observed. AbstractCytomegalovirus (CMV) was isolated concurrently from an infant and her mother on the 2nd day after delivery. Restriction endonuclease cleavage analysis of viral DNAs revealed that these two CMVs were identical. Sera from the infant and mother were both positive for IgM specific to CMV and antibody against early antigen (EA). Thus, virological, serological and molecular analysis indicated the transmission of CMV from a mother to her fetusin utero.

AbstractWe report the case of a male infant with a 46, XY, r(9)(p24.1q34.3) karyotype.
