Stenosis of the tracheostome is an occasional complication following laryngectomy. Having tried a number of procedures recommended for correction of such stenoses with limited results, we developed a new technique. This technique which is based on multiple Y‐V plasties has been applied to 20 patients. Measurements of the stomal area following surgery in 18 patients showed an average increase in stomal size by a factor of 3.8. One patient apparently had no increase of the stoma. In 2, no post‐operative measurements were available. Post‐operative infection resulted in recurrence of the stenosis in one patient. He was successfully reoperated using the same technique. In 169 children with a runny nose the difference between the group with rhinitis and those diagnosed as having sinusitis was indistinct. History and examination, and the results of radiology, ultrasound, the appearance of the sinus washings, the number of leucocytes in the washings, and the presence of pathogenic bacteria in the culture of the secretion from the nose and sinuses had little correlation with each other or with the clinical picture. In our view there is no marked difference between rhinitis and simple sinusitis, that is, mucosal inflammation of the maxillary sinus. This difference is also of no therapeutic relevance, and both diseases may be regarded as part of an upper airway infection.It is important to distinguish between sinusitis with and without empyema, because empyema must be treated differently. The latter disease produces distinct symptoms in the acute stage resembling ethmoiditis in children or maxillary and frontal sinusitis in adults. This type of sinusitis did not occur in our patients. Further investigation in children with a runny nose is indicated only in those with typical symptoms of an empyema.

Clinical voice testing forms part of the routine assessment of children's hearing. However, its role in paediatric practice has not been defined. In particular, the sensitivity and specificity of the test in detecting the presence of a hearing impairment is not known.The value of the test as a screen was assessed by comparison of the results of free‐field clinical voice testing with pure‐tone audiometric thresholds in 141 children aged 5–12 years.In order to pass the screen, a child had to respond to a whispered voice 2 feet (60 cm) from the test ear. The mean pure‐tone average (average at 0.5, 1.0 and 2.0 kHz) of those ears that passed was 14 dB HL compared with 31 dB HL for those that failed (P<0.001). The test was 80% sensitive (95% specific) in detecting a hearing impairment ≥ 25 dBHL, and 89% sensitive (90% specific) in detecting a hearing impairment ≥ 30 dB HL.Therefore, clinical voice tests will fail to detect a significant proportion of children with a hearing impairment and cannot be advocated as a screening method. The clinicopathological features of 10 adult patients with nasopharyngeal tuberculosis (TB) are presented. 9 patients had no evidence of chest or systemic disease. 7 patients presented with cervical lymphadenopathy, and only 4 had nasal symptoms. Examination of the nasopharynx showed no abnormality in 2 patients, lymphoid hyperplasia in 4 patients, and a tumour‐like mass in 4 patients. These findings suggest that nasopharyngeal TB may occur more frequently as part of an isolated upper respiratory tract infection than as secondary to pulmonary infection. The nasopharynx may be a portal of entry for tubercle bacilli in patients who develop cervical lymphadenitis. Involvement of the nasopharynx by TB may be underdiagnosed because it does not produce obvious symptoms or physical signs in all case

Recent research on inflammatory sinus disease has implicated a central role for the ethmoid labyrinth, which may influence changes in the maxillary and frontal sinuses. CT can provide excellent definition of the paranasal sinuses and particularly the ethmoids, which is a prerequisite for endoscopic surgery. We describe a prospective series of 110 coronal CT scans performed on patients with a clinical diagnosis of sinusitis who had undergone diagnostic nasal endoscopies and medical treatment prior to surgery. 86% of the scans showed abnormal mucosal thickening. The ethmoids were affected in 73% and the maxillary sinus in 64%. Pneumatization of the middle turbinate was a common variant and when present was associated with anterior ethmoid disease in 60% of patients. Anterior ethmoid inflammatory changes were demonstrable in 95% of patients with maxillary sinus disease.Direct coronal CT can readily demonstrate disease in the infundibulum, frontal recess and posterior ethmoids in the same orientation confronting the endoscopist, and helps in the planning of ethmoidal surgery. Following surgery the sinuses can be directly inspected in outpatients which reduces the need for any further plain radiographs. It is important that the diagnosis of sinusitis is not based on CT findings alone as isolated areas of mucosal thickening are common in the normal population.
