Surgical approach to the diagnosis of tumours occurring in the superior mediastinumThe nature of tumours lying in the superior mediastinum can frequently be decided by the radiologist using a range of techniques varying in sophistication from the plain chest radiograph to transvenous biopsy. Histological proof of such lesions usually requires surgical exploration and of the methods available it is believed that mediastinotomy is the safest and most rewarding in terms of positive results. Radical excision of tumours and management of tracheal lesions require the greater surgical access afforded by thoracotomy, median sternotomy and the sternal resection of Grillo.8

Branchial cysts, sinuses and fistulaeBranchial cysts, sinuses and fistulae are reviewed with special reference to their origin, clinical features, pathology and treatment. The material consists of a survey of over 700 recorded cases, a series of 90 cases studied in retrospect with special reference to the pathology, and a personal experience of 42. It is suggested that a proportion of ‘branchial cysts’ do not arise from the branchial apparatus, but possibly may do so from epithelial inclusions in lymph nodes. It is acceptcd that branchial sinuses develop from the branchial apparatus, but there is little proof that such an entity as ‘branchial fistula’

Middle ear implantation: its place in the immunohistophysiology of lymphoid tissueThe concept of immunotolerance in otological tissue grafting is presented against the background of some fundamental aspects of immunohistophysiology. Available experimental and clinical data on tympanoplasty with allografts are evaluated in relation to this phenomenon of abolished immune reactivity.
