Successful methotrexate therapy in oral florid papillomatosisOral florid papillomatosis is a rare syndrome, which was first described under this name by Rock&Fisher in i960. The same syndrome had been previously described as papillomatosis mucosae oris carcinoides by Scheicher‐Gottron (1958). It is a benign condition, capable of malignant degeneration. Surgical excision and electrocoagulation are the most widely used therapeutic methods, despite the fact that relapse of symptoms after treatment by these methods is common. The literature comprises a few reports on favourable therapeutic effects obtained with small doses of methotrexate. This paper describes a patient with a multifocal, very extensive form of oral florid papillomatosis, in whom both surgical excision and electrocoagulation were regarded as inadequate in view of the extensiveness of the lesion. Chemotherapy with methotrexate proved to be effective and resulted in complete remission. No relapse has occured during a follow‐up period of over 2 years. Air versus water caloric testThe air caloric test is favoured because of the advantage of higher convenience for patient and operator and its applicability in situations where water is contradicted. We examined 20 normal subjects with air (60 s, 12 min, 45o/29oC) and 25 with water (30 s, 30 cm3, 440/300°C) caloric test. Additionally, we evaluated the influence of flow rate and irrigating tip position on the temperature near the tympanic membrane during the irrigation, using a polyacryl copy of the external auditory canal. A high flow rate and a reproducible tip position–close to the tympanic membrane appeared to be essential to achieve reliable results in air caloric test. Furthermore, measurements of the actual temperature should be done at the irrigation tip delivery in every air caloric test. The lower heat capacity of air, turbulences near the tympanic membrane and air aspired from the environment are the main reasons of more distinct temperature drops in the air caloric test. We observed a significant(P<0.05)difference between maximum slow phase velocity in air (11.5oC/s) and water (19.50C/s) irrigation. The relative standard deviation was markedly higher for air irrigations. Therefore, in spite of the advantages of the air caloric test, the disadvantage of a higher variability, even when using a sophisticated technique, has to be considered.ResuméThe main advantage of air caloric test seems to be its applicability in those cases where water is contradicted. From our subjects greater convenience was not reported, probably because of the high flow rate. As the test‐retest variability and relative standard deviation is higher for air irrigations, the convenience of the whole technique appears of no overall advantage over water calorics. Additionally, measuring the temperature near the tympanic membrane is highly necessary.Because of the variable correlation between air and water responses only an approximate comparability can be achieved, if the above mentioned factors are considered. Only by using a very correct test procedure (for description in detail see ref. 4, p. 219) and a quite accurately controlled technique, can sufficiently comparable results be achieved.Bearing in mind these considerations, the air caloric test appears to be a useful tool, mainly in those cases where water cannot be applicated.
