Dizziness in childhood is not an infrequent symptom. Accurate history taking and close co‐operation between otologist, paediatrician and neurologist are necessary in the approach to the dizzy child. Most cases of childhood dizziness settle in time and investigations should be carefully selected; those with severe and persistent dizziness or ataxia should be thoroughly investigated including: EEG, ENG, calorics and CT scan. The conditions causing dizziness in children are discussed and are illustrated with case histories from our series of 27 children. Dizziness of unknown aetiology, serous otitis media and benign paroxysmal vertigo were the most common diagnostic labels applied to our patients. Treatment is rarely necessary but dimenhydrinate or a labyrinthine sedative in those with troublesome vertigo, or the adjustment of the medical regime in those epileptics on phenytoin, may be beneficial. The resonance qualities of the external auditory canal can increase the sound pressure level (SPL) at the tympanic membrane by as much as 20 dB between 2 and 4 kHz. Surgery on the canal, particularly in the form of a modified radical mastoidectomy, might be expected to affect this with resultant implications for speech discrimination. The creation of a modified radical mastoidectomy in 6 temporal bones altered the SPL at the tympanic membrane by + 10 dB at 2 kHz, + 14 dB at 2.5 kHz, ‐ 2 dB at 3 kHz, ‐ 12 dB at 3.5 kHz, ‐ 20 dB at 4 kHz, ‐ 16 dB at 5 kHz and ‐ 8 dB at 6 kHz. When this transfer function was applied to 2 subjects, speech discrimination in noise was reduced by 6%. One hundred and seventy‐four patients with early glottic cancer received radiation therapy on the kV250 and cobalt 60 machines. Crude survival rates at 5, 10 and 15 years were 83%, 73% and 60%, respectively. Salvage surgery for locoregional recurrences was successful in 85% of the patients. A significantly higher failure rate was noted in patients with a pre‐existing chronic laryngitis, with lesions confined to the left vocal cord and in those treated with KV250 irradiation. The rate of second primaries in this case material was 11%, 70% of which were noted in the respiratory tract. Megavoltage radiotherapy offers excellent chances for cure in T1laryngeal carcinoma with preservation of good voice in most patients. The use of chemoprophylaxis in major Head and Neck surgery is controversial. Prior to conducting a prospective trial a controlled microbiological monitoring of 32 patients was performed to determine a suitable antibiotic for prophylaxis. Wound infection occurred in 22 (68%) patients with fistulae occurring in 10 of these patients (45.5%). Pre‐operative cultures were of limited value in predicting subsequent sepsis. From 1964 to 1980 28 patients with extensive paragangliomas were treated with radiation therapy. Nine patients had a carotid body tumour and 19 had a temporal bone tumour. During the 16 year period the radiation dose was gradually reduced from 60 Gy in 6 weeks to 40 Gy in 4 weeks. Ten children with histiocytosis X and recurrent otitis media as a presenting symptom were compared to children with otitis media complicating cholesteatoma. When radiography and biopsy findings were not diagnostic a low age (<3 years) and a remaining elevated ESR were characters that most effectively separated the histiocytosis X group from the cholesteatoma group.

The four surgical methods currently in use to improve hearing in patients with congenital aural atresia are (i) fenestration of the lateral semicircular canal, (ii) type III tympanoplasty, (iii) canal plasty and (iv) canal plasty with homograft tympanoplasty. Because the surgery is difficult and the functional results are not always good, the criteria for surgery are also influenced as to whether the anomaly is unilateral or bilateral and what the cosmetic goal is. When aural rehablitation and cosmetic surgery are contemplated, co‐ordinated planning by the otological and plastic surgeon is necessary to attain the best end result. Of the 36 ears with a type II anomaly operated on at the University of Nijmegen between 1972 and 1983 by the technique of canal plasty, the average hearing gain was 20 db, with 18 patients having a 35 dB or better air conduction threshold. The functional results seem to be related to the degree of malformation involved. Classification of the malformations has proved to be useful in discussing the surgical result
