The CT scanner and the recognition of anaerobic organisms has altered the investigation and management of intracranial suppuration. Improved treatment of acute and chronic middle ear infection has also occurred. A 15 year retrospective review was undertaken to assess the effects of these changes. The ears and sinuses were confirmed as the major source of intracranial suppuration (69%) although initial presentation to ENT surgeons was less common (14%). Ear complications occur in childhood and in the fifth and sixth decades; sinus complications predominantly in the second and third decades. The incidence of sinus infection appears to be rising. With the introduction on the CT scanner cerebral bscess mortality declined from 27.5% to 6.5% and subdural abscess mortality halved. The introduction of metronidazole contributed to a drop in mortality. The use of the CT scanner and more accurate antibacterial treatment, including metronidazole, gives hope for a more successful outcome than was previously possible.

This study gives the results of a series of studies on inner ears carried out with the help of microdissections and surface preparations. The total degeneration patterns of the cochlea of the guinea pig due to two different ototoxic drugs were investigated with this technique. Clear differences between the results of the ototoxic drugs (gentamicin and cis‐platinum) in the inner ear were observed. Gentamicin has a ‘typical starting point’ of degeneration on the basilar membrane. From this ‘degeneration point’ the loss of hair cells progressed towards the round window (fast) and the apex (slowly). The stria vascularis showed no signs of degeneration due to gentamicin. Reissner's membrane, on the other hand, showed cellular vacuolization of the endolymphatic cells. Cis‐platinum (DDP) showed no ‘degeneration point’; the loss of hair cells was found over the complete length of the basilar membrane, with a preference for the basilar turn. The stria vascularis, on the other hand, showed severe degenerative changes due to DDP, whereas Reissner's membrane showed no change. It has previously been demonstrated that hair spray sprayed onto the hair of experimental subjects reduces the mucocillary transport in the trachea and nose. Ladies hairdressers are exposed to the effects of hair spray throughout the whole working day, and we therefore investigated the nasal mucociliary transport of 60 hairdressers and 15 control subjects before and after a working day by the saccharin method. We found a significant reduction in the mucociliary transport of the ladies hairdressers during the working day (P<0.001), but not in control subjects. The reduction in mucociliary transport could be correlated to the amount of hair spray applied by the individual hairdresser to the hair of the customer during the day.

Fifty‐five consecutive patients had ultrasonic examination and routine radiology of the maxillary sinus prior to proof puncture. One hundred and eight antra were washed out and a positive aspiration was obtained in 20% (22). A clear return correlated with a normal radiograph and ultrasound in 91% and 85% of sides respectively. Radiographic results gave a 9% false negative result whereas 15% of normal ultrasounds had a return on proof puncture. Abnormal radiographic appearances and ultrasound traces were correlated with a positive return in 28% and 27% of punctures, respectively. A practical procedure is described for obtaining frequency specific information with the auditory brain stem response (ABR). ABR thresholds were determined with a 4 kHz sine wave and with a 1 kHz sine wave combined with high pass filtered masking noise (cut‐off 2 kHz). The differences between these 2 thresholds were compared with the slopes of the tonal audiogram, in a sample of 30 ears with various kinds of perceptive hearing loss. The medical records of 1150 tonsillectomized patients were retrospectively reviewed for registration of the frequency of post‐tonsillectomy haemorrhage in relation to sex and age of the patients, the indications for tonsillectomy, and the operative experience of the surgeon. The number of post‐operative bleeds requiring surgery was 32 (2.8%) and occurred most frequently in young men and in patients with a history of previous peritonsillar abscess undergoing cold tonsillectomy. The frequency of haemorrhage in abscess tonsillectomy was not higher than expected. As previous studies have demonstrated that abscess tonsillectomy is associated with a minimal risk of spreading the infection or other serious complications, it is suggested that abscess tonsillectomy should be the preferable treatment of peritonsillar absc

The symptoms and signs as well as the bacterial flora from the auditory canals of 54 Tanzanian patients (74 ears) living in Dar es Salaam and suffering from otitis externa were studied. Itching and pain were the most common symptoms and erythema and secretion were the prevalent clinical findings.Pseudomonas aeruginosawas found in 38%,Aspergillusin 24%,Staphylococcus aureusin 18% andCandida albicansin 14%. Bacterial cultures from the auditory canals of 21 healthy subjects revealed only normal skin flora.

The various techniques available for the management of auricular haematoma are reviewed and discussed. A modification of existing methods of treatment employing vacuum drainage is described. This method used in six cases resulted in a satisfactory cosmetic appearance.

A 6 year personal experience in the performance of Plastipore prostheses in middle ear reconstruction is presented. The good hearing results at 12 months are not maintained with TORPs or PORPs and 41.6% have been rejected. Better results are obtained with a tube prosthesis between a mobile malleus and stapes. The use of TORP and PORP has now been abandoned in favour of ossicles or homograft cartilage.
