In 1983, 1338 liveborn infants with a gestational age of less than 32 weeks and/or a birthweight of less than 1500 g, were enrolled in a national follow‐up study in The Netherlands. At the age of 5 years, 966 children were alive. Of these, 927 (96%) were assessed on a home visit 2–6 weeks after their fifth birthday by three specially trained paediatricians. An assessment of ENT morbidity was made and compared with ENT morbidity in full‐term children of the same age group. A group of 266 children (515 ears), ranging in age from 5 months to 11 years, was studied. These children were candidates for the insertion of ventilation tubes, or adenoidectomy and/or tonsillectomy with myringotomy. Before surgery, tympanometry was performed. The surgical and tympanometric findings were compared afterwards. Two different tympanometers were used (GSI‐27A and TYMP‐85TT). This study showed a comparable validity of these two tympanometers. The sensitivity and specificity of tympanometry in the age group of 5 months to 2 years did not show a significant difference from that in the age group of 2–12 years. The best anastomotic agent to join nerve ends is controversial. This paper describes a controlled trial between a collagen tube wrap and a tissue glue to anastomose the rat facial nerve. There was no difference in the results using photographic, histological and electrophysiological techniques.

Obesity, short stature, hypotonia and excessive daytime sleepiness are characteristic features of the Prader‐Willi syndrome. Excessive daytime sleepiness has been attributed to obstructive sleep apnoea (OSA). To investigate the role of anatomical factors in OSA in the Prader‐Willi syndrome, clinical and ENT assessment, radiology of the upper airway and polysomnography including sleep oximetry were done in 14 subjects. Excessive daytime sleepiness was present in eight of 14 subjects as determined by a mean sleep latency to non‐rapid eye movement stage I–II of9 (Epworth Sleepiness scale). Seven subjects were snorers or mouth breathers and dental abnormalities were present in 11. Sleep apnoea, as determined by a combined apnoea‐hypopnoea index of more than 10 respiratory events per hour was present in 12 of 14 subjects.On clinical assessment, the nasopharynx, oropharynx and hypopharynx were small in one subject. No subject had redundant pharyngeal mucosa or an enlarged tongue. However, radiological studies performed in the awake supine posture showed a slight reduction in the cross‐sectional area in nine subjects at the oropharyngeal level and in four subjects at the nasopharyngeal level as compared with normal control subjects.Sleep apnoea and minor radiological evidence of narrowing of the upper airway are common in the Prader‐Willi syndrome, although clinical otolaryngological examination is often unremarkable. Hearing conservation in acoustic tumour surgery remains controversial. There have been few previous reports in the British literature. The senior author has managed 24 patients by retrosigmoid surgery with the intention of preserving hearing during the last 9 years. The clinical features, surgical technique and results are discussed with respect to pre‐operative selection criteria, and post‐operative quality of hearing. Hearing preservation has been achieved in 11 (78.6%) of 14 patients with small or intracanalicular tumours and a mean minimum auditory threshold of 35 dB and 70% speech discrimination, the majority (81.8%) above the 50 dB/50% level. Tumour filling the fundus of the internal auditory canal was found to be a significant adverse prognostic factor as regards successful hearing preservation. Tumour excision was complete in all patients. Nearly 90% of patients had normal facial function, and the remaining 10% grade II function. Associated morbidity was minimal. It is desirable to screen snoring patients for obstructive sleep apnoea (OSA) prior to surgical treatment. We postulated that the addition of a sound profile would increase the value of overnight oxygen saturation (SaO2) as a screening method. Thirty‐nine polysomnographic studies including sound level measured by calibrated meter were performed on snorers being considered for uvulo‐palato‐pharyngoplasty (UPPP). Polysomnography showed an apnoea/hypopnoea index (AHI) ≥ 15 per hour of sleep in seven subjects. Two experienced observers independently, without knowledge of other data, classified paper records of SaO2alone and SaO2plus sound level obtained during polysomnography as OSA ‘unlikely’, ‘equivocal’ or ‘definite’. The addition of sound to SaO2reduced the number of equivocal results from 14 to six and increased the number classified as ‘definite’ or ‘unlikely’.
