The aetiology of pharyngeal diverticula is unclear. There are only a few case reports of pharyngeal pouches occurring in families in the literature and the importance of inheritance has been relegated to ‘other proposed aetiological factors’ in a recent paper. 7 We studied the incidence of a family history in all patients diagnosed as having pharyngeal pouches between 1980 and 1985 in the Leeds/Bradford area. Of 32 people questioned, 7 had died leaving no relative to provide the information. Five patients (20%) recorded a positive family history and 20 patients (80%) a negative history. This finding is of relevance to the controversy surrounding cricopharyngeal myotomy in combination with pouch excision. Auditory brainstem responses (ABR) and transtympanic electrocochleography (ECochG) were analysed for 63 patients with tumours of the cerebellopontine angle (CPA) and/or internal auditory meatus (IAM). ABR recordings indicated a clearly prolonged wave I‐V interval (above 4.3 ms) in half of the patients, hence suggesting a retrocochlear disorder. For the remaining patients ABR alone was not sufficient for clear diagnosis and ECochG was therefore used in addition. When the latency of wave V was uncertain due to the absence of wave I, the latency of wave Nl was always measurable by ECochG. The NI‐V interval evaluated in this way was always significantly prolonged (above 4.5 ms) relative to a control group of patients with a sensorineural hearing loss. In 9 of the 13 subjects without any discernible ABR, an ECochG response could be recorded and its threshold was often better than the mean pure tone audiogram. This study confirms the validity of transtympanic ECochG in the diagnosis of retrocochlear disorders. A consecutive series of 57 patients who had undergone an operation on the nasal septum was followed up 1.5–3.5 years postoperatively. Rhinomanometric measurements, a clinical examination of the exterior of the nose, and rhinoscopy were performed. These same examinations had been carried out preoperatively and the results recorded preoperatively. The results of the preoperative rhinomanometry were not known to the surgeon and the indication for surgery was based on clinical examination alone. At follow‐up the patients also answered a questionnaire. According to the findings at rhinomanometry 52 of the 57 patients had a better airway after the operation. In 11 patients who subjectively did not consider the nasal airway to have been improved by the operation, an allergy or vasomotor rhinitis was found and in some cases the results of preoperative rhinomanometry (total nasal resistance) were normal. To measure the error of the method, a separate series of duplicate measurements was performed on 50 subjects. The standard deviation of the difference between duplicate measurements was found to be 15% of the mean value and the error of the method was considered small enough to justify rhinomanometry as a method for inter‐individual comparisons, provided the laboratory standard is well

Medicament contact dermatitis can be troublesome in external ear disease. In this work the effects on the guinea pig external ear of instillation twice daily for 3 weeks of benzalkonium chloride, Cerumol, gentamicin, the pharmaceutical base for Gentisone HC, hydrocortisone and propylene glycol were studied using light microscopy, thickness measurements of tympanic membrane and meatal epidermis, and the mitotic index of meatal epidermis. Benzalkonium chloride induced severe inflammatory changes and markedly increased the thickness measurements and meatal mitotic index. Cerumol also increased the thickness measurements and the mitotic index; there was additionally low grade chronic inflammation in the dermis. Prophylene glycol increased the mitotic index and thickness measurements though no signs of inflammationwereseen. Other compounds did not produce significant changes except that hydrocortisone reduced the mitotic index. Contact dermatitis to benzalkonium chloride may be important in persistent ear disease. Cerumenolytic agents ought not to be used for prolonged periods or as prophylaxis.

All medical schools include undergraduate ENT teaching, but the time allocated is short and the aquisition of practical skills may suffer. Efficiency is essential, particularly with regard to those destined for general practice, where 10–30% of the workload concerns the upper respiratory tract. The students at the Leeds Medical School, during 1 academic year, were surveyed by questionnaire on various aspects of their ENT course. Criticisms and suggestions for improvement were also invited. The results suggested the lecture course should not change but the time spent in operating theatres could be reduced in favour of more time in the outpatient clinics with a greater emphasis on practical skills. This discussion was then extended to consider what an ENT course should include and whether it could be integrated, in some way, with postgraduate educatio

Several publications have addressed the problem of atrophic rhinitis and ozaena following turbinate resection for chronic nasal obstruction, but little attention has been given to the problem of intranasal adhesion formation following this procedure. To determine the incidence of postoperative intranasal adhesion formation, a retrospective study of 479 operations on the inferior turbinate and nasal septum, either alone or in combination, was carried out. Adhesion formation complicated 36.2% of turbinate resections, 30.6% of turbinate resections in combination with septal surgery and only 6.8% of all the other procedures. There was also a significantly higher incidence of postoperative haemorrhage following turbinate resection (7.4%) than that following other procedures (1.9%). There was no noted occurrence of atrophic rhinitis, rhinitis sicca or ozaena.

Screening for acoustic neuromas in large numbers of patients in whom the possibility exists should effectively select out the small proportion who have a tumour, prior to more definitive but costly radiological scanning techniques. A suitable screening protocol has been suggested using a combination of caloric testing, tomography of the internal auditory meati and evoked response audiometry. The protocol was applied retrospectively from the records of 64 patients with proven acoustic neuromas and found to predict correctly the presence of a tumour in 97% of cases. It is concluded that this combination of investigations constitutes an effective method of screening patients in whom an acoustic neuroma confined to the 8th nerve is suspected. The great importance of early diagnosis is that the operative mortality and morbidity is much less when the tumour removed is small.
