The acoustic brainstem response (ABR) in clinical use is generally elicited by a wide band click stimulus. This gives a well‐synchronized response detectable at low sound intensities, but requires tedious biological recalibration which could be largely eliminated by the use of pure tone pip stimuli. We have therefore assessed ABRs in response to pure tone pips at 2 KHz and 4 KHz. This has shown that these tone pips elicit a much poorer response than clicks, having thresholds which are both significantly higher, and more widely distributed. Twenty‐one patients with nasal obstruction due to allergic or vasomotor rhinitis were assessed rhinomanometrically before and after the operation of submucosal diathermy to the inferior turbinates. Computer templates of the action potential/summating potential waveforms from electrocochleograms have been analysed in the frequency domain following the Fast Fourier Transform. Analysis in the frequency domain is compared with standard time domain analysis and some inadequacies of the latter technique are shown. A simple account of the principles of Fourier analysis is given which may be of assistance to the general reader, particularly as computer packages currently being marketed with electric response audiometry equipment make this analytical technique readily available.

Although autostapling for the gastrointestinal tract is well established, some doubt exists over whether such techniques are suitable for the closure of diverticula cosisting of mucosa alone. In a prospective investigation of the treatment of glue ear in children, the possible role of persistent infection in the tonsils and adenoids was assessed by comparing bacterial cultures of swabs and resected tissue from the tonsils and adenoids of patients with cultures of swabs from similar sites in control subjects without ENT abnormality. For almost all potential pathogens, includingStreptococcus pneumoniasandHaemophilus influenzae, no statistically significant differences were demonstrated between patients and controls. The same was true ofStreptococcus pyogenesin tonsil specimens, while in adenoid specimens rather more isolates were obtained in patients than controls. This difference was just statistically significant at the 5% level, but only when all isolations, including very scanty growths, were compared. On present evidence, persistent infection should not be invoked to explain the success of adenoidectomy in otitis media with effusion or to justify adenotonsillectomy.

Twenty consecutive patients visiting an otolaryngological department in Sweden with a facial palsy were investigated for serological signs of tick‐borne spirochete infection. Four patients showed serological evidence of and had a medical history compatible with a tick‐borne spirochetosis. In order to evaluate the indications for massive surgery (maxillectomy and excentera‐tion of the orbit) in carcinoma of the maxillary sinus, 96 patients were reviewed in this retrospective study. The absolute five‐year survival in those patients treated for cure and with at least 5‐year follow‐up was 51%. The survival in patients with close or positive surgical margins, with invasion of the anterior cranial fossa, skin or soft tissue of the cheek and with cervical lymph nodes on presentation was reduced considerably.
