For endolaryngeal laser surgery an anaesthetic technique is required which provides the surgeon good access to the larynx. High frequency ventilation of the lungs using a FG 14 insufflation catheter covered by aluminium foil has proved to give a nearly optimal condition for the surgeion, while the fully paralyzed patient is very well ventilated. This has been used in more than 1000 patients, 200 of them have been treated by the laser beam in the airway. A method of total intravenous anaesthesia is described using short acting drugs. This way early awakening after surgery in the airways prevents aspiration of blood or debris into the lungs.

The incidence of protein and calorie malnutrition in bucco‐pharyngeal cancer patients is high and therefore nutritional support is indicated. Two methods of tube feeding, fractioned enteral (FEN) versus constant rate enteral nutrition (CREN) have been compared in a prospective study. Based on anthropometrical, biological and immunological criteria, patients on CREN showed a significant improvement of their post‐ooperative protein state in comparison with patients on FEN. This difference was due to a better tolerance to CREN, which allowed a more appropriate protein and calorie intake. Furthermore the time of intravenous infusion was reduced in patients on CREN. A review is given of the development of the concept of an artifical tympanic membrane. Starting with homologous tympanic membranes we compared biodegradable collagen materials (homologous and heterologous) and biodegradable synthetic materials, poly‐glycolic acid, poly‐lactic acid and poly‐α‐amino acids. In this paper we report on the prevalence of clinical otosclerosis in South Australia, and discuss some aspects of its distribution and character. Our data comes from an exhaustive search for stapedectomy patients in a relatively closed population within a fixed time span, and hence our figures represent a lower limit for the true prevalence and not an unbiassed estimate. Nevertheless, even with no allowance for this downward bias it woulld seem that South Australia has a high prevalence of otosclerosis by world standards, particularly of obliterative cases. The highest prevalence is about 5 per 1000 of population for men and 10 per 1000 of population for women, which occurs in the 50‐59 year old age group. Proplast and Plastipore prostheses, removed at revision surgery, have been examined under the electron microscope. Foreign material in the multinucleated foreign body giant cells, previously reported under the light microscope, has been confirmed. The appearances were consistent with the conclusion that the giant cells were breaking down the prostheses by two different modes of action. A comparative study was carried out in 118 patients suffering from seasonal allergic rhinitis in order to evaluate possible differences between two different preparations of budesonide with regard to effect and adverse reactions. The glucocorticoid was supplied either as a freon propellant device (Rhinocort) or as a water solution in a mechanical pump spray. The freon aerosol was administered in a daily dosage of 400 μg. The water solution was administered in daily dosages of 400 μg or 200 μg. The patients scored their nasal and ocular symptoms daily for 1 month, and pollen counts were registered over the same period. The drugs employed in the study provided good control of the nasal symptoms. The daily dosage of 400 μg in both preparations proved more efficacious than the daily 200 μg dose in the nasal pump spray.
