Two cartilage autografts used for ossiculoplasty for 3 and 8 years were examined under the transmission electron microscope. The most significant change was progressive deposition of calcium salts in the matrix of the cartilage. Chondrocytes showed degenerative changes in the nuclei, cytoplasm, and cell walls. Contrary to common belief, autograft cartilage did not seem to resorb in time.

Preservation of the fine structures of the human cochlea has been achieved by perfusing the cochlea with fixative shortly after death. Following the dissection of the temporal bone the surface of the organ of Corti and stria vascularis has been examined in the scanning electron microscope. The surfaces of the inner and outer hair cells can be seen and the stereocilia projecting from their surfaces closely examined. The number and length of the stereocilia of the outer hair cells changes linearly with distance along the cochlear duct. The surface of the stria vascularis is similar to that seen in other animals.

A comparison of therapeutic results in surgery of chronic otitis media is impeded by the absence of a well defined and generally accepted system of documentation. The prerequisites of such a system are described, and a model of a system that fulfills these prerequisites is presented and discussed.

Two cases of chemodectoma of the larynx, I benign and I malignant, are reported. Initially both tumours caused diagnostic problems but the diagnosis could be confirmed by silver staining and electron microscopy. So far only 25 cases of laryngeal chemodectoma have been reported in the literature. This study is based on a very large series of histological sections of non‐tumour‐bearing larynges, most of them in the horizontal plane. Six hundred sections in one larynx were performed to show the constitution of the anterior region of the larynx at all levels. Although the same tissues are present (fibres, glands and vessels) in almost the entire extent of the anterior segment of the larynx, there is a small zone immediately above the vocal cords with neither glands nor vessels. The authors designate this zone as ‘plane O'which separates the anterior commissure from the thyro‐epiglottic ligament. It is not possible to separate the anterior commissure from the most anterior portion of the subglottis, because the glands and vessels are in continuity. A new method of surface electromyography (EMG) is presented, for sampling the posterior crico‐arytenoid, interarytenoid and crico‐pharyngeal sphincter. The results obtained in normal volunteers are discussed and the indications for laryngeal electromyography (LEMG) outlined. It has the advantage of being non‐invasive and safe, and can be done as an out‐patient procedure with minimal discomfort to the patient, and unassociated complications. This has provided a new and fascinating field of investigation in neuromuscular disorders of the larynx. The effect of the use of electric response audiometry (ERA) on the diagnosis and management of children has been investigated. A retrospective method was employed using the notes of the first 102 hearing assessments in 1978 where an ERA test was performed. The results show that the effect on diagnosis has been to improve the degree of certainty rather than to re‐classify patients as deaf or normal. The effect on management has been to reduce by 24% the number of patients that require repeat assessment. These effects are more pronounced in the ‘0‐1 age group’ as opposed to the ‘above 1 year
