The laryngograph was used to record and compare the voice changes in 11 thyroidectomy patients and 14 controls. The frequency histogram was studied and the change in its spread following surgery used to identify abnormal vocal fold movements due to recurrent laryngeal nerve damage. It also demonstrated changes which occur in thyroidectomy patients and not in patients having other operations. The possible causes include external laryngeal nerve damage.

A matched‐pair parallel, group trial was carried out on 2 groups of 13 subects (26 subjects in total), using 2% aqueous lignocaine to anaesthetize both the nasal vestibule and the nasal mucous membrane. Both groups had lignocaine infiltrated into the nasal vestibule and lignocaine applied topically to the nasal mucous membrane. The groups differed in the order in which the medication was given, one group having the infiltration first and the other group the topical application first. Local anaesthesia of the nasal vestibule caused a marked decrease in the nasal sensation of airflow (P<0.05) with no accompanying change in nasal resistance to airflow. Topical anaesthesia of the nasal mucous membrane had no effect on either nasal sensation of airflow or nasal resistance.The study demonstrates that the nasal vestibule is the dominant area for sensing nasal airflow. The nasal cavum (the major part of the nasal cavities that is lined by respiratory mucosa) appears to be unimportant as regards the sensation of nasal airflow.The clinical significance of the study is discussed with reference to other work, in this fiel

Modern techniques have reduced the long existing reluctance to perform lingual tonsillectomy. The results of treatment of hyperplasia of the lingual tonsil with the carbon dioxide laser in 32 patients are presented. After exposure with a special endoscope the lingual tonsil can be vaporized or excised under microscopic control with promising results, far superior to the results obtained with other surgical techniques. Since the success rate of the surgery depends on the diagnostic criteria for applying this therapy, a careful selection of patients is essential

This study describes the histological features of the tympanic membrane which has been in contact with an indwelling ventilation tube and considers the factors of tube type, timein situ, persistent aural discharge and the comparison to perforations residual to grommet extrusion. The specimens were obtained at the time of operative removal of a grommet. The 21 margins examined were characteristically hyperplastic and hyperkeratotic with a prominent granular layer. The squamous epithelium was from 8 to 25 cell layers thick and the connective tissue core was increased. Chronic inflammation was common. Features of chronic otitis media (respiratory epithelium, goblet cells and glandular tissue) were found in 7 margins (2 perforations and 5 grommet specimens). A theory of grommet extrusion is persented in this paper in the light of the histological findings.

Percutaneous needle laryngeal electromyography was used in 22 patients with an established vocal cord palsy of non‐laryngological cause. Recordings of action potentials from both vocalis and cricothyroid muscles produced expected results in 8 patients with skull base lesions. In 6 patients with known lesions below the clavicles, ipsilateral cricothyroid activity was found to be normal despite the vocal cord lying in the lateral (or cadaveric) position. Temporal bone studies have demonstrated that a modified radical mastoidectomy changes the resonant characteristics of the external auditory canal, but the effect has not been reported in patients. In 12 patients, performing open‐cavity mastoid surgery for cholesteatoma changed the mean peak resonant frequency of the external ear canal from 2.5 to 2.2kHz (p<0.02). This is in comparison with creating a modified radical mastoid cavity in 6 temporal bones which changed the mean peak resonant frequency of the external ear canal from 3.9 to 1.9 kHz. Between October 1984 and December 1987, 219 patients who fulfilled the criteria for Acquired Immune Deficiency Syndrome (AIDS), as set out by the Centers for Disease Control, Georgia, were seen at St Mary's Hospital. London. The most common otolaryngological manifestations of the disease in these patients were oral candidiasis (60%), oesophageal candidiasis (16%), persistent generalized lymphadenopathy (27%) and Kaposi's sarcoma (26%). Our management of the patients is described.

The records of 231 patients with laryngeal papillomas were reviewed. Considering age at onset and sex distribution, the patients were divided into four groups: juvenile solitary, juvenile multiple, adult solitary, and adult multiple papillomas. The main conclusions are (1) it is in general impossibl to predict the course in individual cases, but solitary papillomas in adults follow a less severe course than do juvenile and adult multiple cases; (2) late recurrences (up to 42 years) makes it impossible to declare a patient cured; (3) there is no linkage between puberty and the disappearance of laryngeal papillomas; (4) a latent infection, acquired at or before birth, is unlikely to be responsible for adult papillomas.

Electronystagmography (ENG) was performed on 127 dizzy patients and the findings were compared with the diagnosis obtained with a comprehensive neurological test battery. ENG was found to be abnormal in 49 (39%) of the patients: 19 with unilateral vestibular hyporeactivity, eight with directional preponderance, 12 with spontaneous or unidirectional positional nystagmus, eight with abnormal smooth pursuit, and 13 with other abnormalities. Among the patients with abnormal ENGs, established central nervous system lesions were found in 28 cases (19 of these infratentorial lesions); nine peripheral vestibular lesions and five undefined vestibular lesions were found. Patients with normal ENGs showed fewer peripheral vestibular lesions and more dizziness of psychogenic aetiology. Almost half the patients with infratentorial lesions had normal ENGs. Patients with roatory vertigo had fewer ENG abnormalities than those with other types of dizziness.
