Thirty‐one patients with a suspected inflammatory or neoplastic lesion of a major salivary gland were investigated with ultrasound, and in 27 of them sialography was carried out in addition. The results of the diagnostic procedures were compared. Lesions of the gland parenchyma and intra and extra‐glandular duct ectasia were demonstrated by ultrasound. Salivary stones were demonstrated by this method and ultrasound allowed assessment of periglandular structures. Sialography is indispensable for detailed assessment of the salivary ducts. The present report shows that ultrasound is superior to sialography in the assessment of salivary gland disease. The anatomy of the parapharyngeal space is described and experience with 23 patients with parapharyngeal tumours seen over a 10‐year period is reported. Despite reaching a large size, these tumours may be relatively asymptomatic, or be discovered incidently. CT scan, MRI, arteriography and fine needle aspiration are useful in establishing a diagnosis. The most common histological type (13/23 patients) was pleomorphic adenoma and these tumours were managed by transoral removal if small, or a combined transoral‐external approach if large. No patient required a mandibulotomy for access and no benign tumour recurred. From a total of 323 adenolymphomas in the files of the Bristish Salivary Gland Tumour Panel there were 20 cases (6.2%) which showed extensive necrosis, fibrosis and inflammation. The clinical and microscopical features of this distinctive subtype are described and it is suggested that infarction rather than infection is the more probable aetiological factor. In 40% of these infarcted adenolymphomas there was granuloma formation which mimicked tuberculosis or sarcoidosis and this possibility should be borne in mind when examining lesions in the parotid region.

Pseudohypacusis is a condition in which there is an apparent hearing loss in the absence of clinical or audiological evidence; indeed, audiological assessment for inconsistencies is the mainstay of diagnosis. It is easier to diagnose in children than in adults, due to a child's inability to produce consistently similar results on repeat tests. We have reviewed 15 children with pseudohypacusis, who had an average loss in the right ear of 57 dB, and of 59 dB in the left ear when tested by pure tone audiometry. Eight children were diagnosed by clinical impression and inconsistent pure tone audiograms but in order to confirm the diagnosis in the other seven children further testing with a speech audiogram was necessary and proved sufficient to establish the diagnosis in all cases. The hearing of all 15 children resolved spontaneously at between 2 and 70 months, when treated by careful monitoring and supportive therapy, and none of the children needed psychiatric referral.

Despite the low incidence of these diseases, both juvenile onset and adult onset laryngeal papillomatosis continue to concern otolaryngologists because treatment is often prolonged and unsatisfactory. Over many years various treatments have met with varied claims of success, yet no treatment today is universally accepted. We review the underlying clinical problems, the presentation, the treatment and eventual outcome of 113 patients seen at this hospital over the last 20 years with laryngeal papillomatosis. Using multivariate analysis of age at presentation, sex, smoking habits, initial site of disease, frequency of treatment and eventual outcome, two distinct patterns are revealed both between juvenile and adult onset disease and between male and female patients. Few objective measures exist to predict the outcome of ear surgery. Clinical and experimental experience has shown the key role of the Eustachian tube in this respect, but clinical testing is seldom performed. The present study confirms that Eustachian tube testing may be of clinical significance by comparing preoperative test results before myringoplasty with the late results of this operation. In 157 patients the ability to equilibrate a static air pressure of plus or minus 200 mm of water passively or actively was tested. It turned out that in those 15% who could equilibrate minus 200 mm passively a 7 times higher rate of succssful myringoplasty was found. Ears with cholesteatoma showed less ability to equilibrate actively. Despite these findings it is concluded that the present tests are insufficient for clinical use.

Thirty‐five patients undergoing a functional septoplasty for nasal obstruction and pathologically high nasal airway resistance were followed up twice, at about 9 months and 9 years postoperatively. The resistance of the preoperatively narrower cavity was reduced at both check‐ups, while that of the preoperatively wider cavity, and of the total nose, was lower at the late follow‐up than at the early one. Seventy‐four per cent of the patients were satisfied with the operation at the first follow‐up, and 69% at the later one. Preoperatively, all the patients suffered from nasal obstruction. Postoperatively, at the 9‐month follow‐up 51% were subjectively free from obstruction, but only 26% were symptom‐free at 9 years. Children with Down's syndrome have a higher incidence of middle ear effusion than normal children. Twenty‐eight patients with Down's syndrome and 33 age and sex‐matched normal controls were studied. They were subdivided into 2 further matched groups on the basis of having normal or impaired hearing. Each underwent pneumatic otoscopy, audiometry and lateral head and neck radiography. This study examines radiological parameters to determine whether skeletal or soft tissue factors within the skull base or nasopharynx could be implicated as a cause or diagnostic feature of Eustachian tube dysfunction. The nasopharynx tended to be smaller in those with Down's syndrome and this resulted in a reduction in the airway size due to encroachment by the soft tissues.
