ABSTRACTI hope to make a preliminary contribution to the development of a model that more clearly reformulates the practices of psychoanalytic psychotherapy in a way that recognizes a diversity of analytic practice that I think has been misdescribed ‐ technically and politically ‐ as ‘psychodynamic’. The paper firstly considers the question of definition and suggests four identifiable meanings of the term’ psychodynamic’. The discussion then reviews a number of core and inextricably connected questions of theory and practice under the following headings: setting and regression, time and frequency, countertransference, creativity and the real relationship, interpretation and pure technique, and the problem of eclecticism. The paper concludes by briefly exploring the parameters of psychodynamic practice in relation to eclecticism in psychotherapy, and makes a plea for a model that is consistent, authentic and

ABSTRACTThis paper explores the ways in which psychoanalytic conceptions of self and personal change suffuse culture and so frame the activities of psychotherapists. It is argued that the modern culture which gave issue to psychoanalysis has mutated in recent years into what some writers have termed ‘postmodernity’, and that psychoanalytic notions have also changed. The conceptual device of the ‘discursive complex’ is used to trace how themes of ‘intellectualization’, ‘transference’ and ‘trauma’ which organize modern therapeutic discourse have been augmented, through the development of new forms of communication technology, by postmodern themes of ‘the Symbolic’, ‘Che Vuoi’

ABSTRACTIn this paper I will be looking at the nature of the transference relationship, and asking whether it can be seen as a template for understanding other relationships outside the analytical setting. I will be positing the idea that there are relationships with profound similarities to the analytic relationship in areas as diverse as the creative arts and family generations. It is my feeling that through a close inspection of creative transference relationships, such as the relationship between a reader and a text, we can begin to understand how dyadic transference relations are formed and acted out. I am also hoping to show that through other transference relationships we can learn to interpret and analyse the feelings of countertransference. I am suggesting that transference and countertransference feelings are not restricted to the analytical setting.

ABSTRACTIn order to understand the significance of countertransference reactions, it is important to separate unexpected and unusual therapist reactions from those that are normal and expected. In this paper, two different perspectives on how to estimate the unusualness of countertransference reactions are presented. The first method compares professionally expected reactions with professionally unexpected reactions. The second method uses the recurrent reaction pattern of the therapist and the recurrent reaction pattern towards the patient to sort out countertransference reactions that can be considered as unusual. The data for the analysis consists of answers by milieu therapists on a check‐list about feelings for patients. The comparison between the methods shows that it is misleading to evaluate a countertransference reaction against a common professional norm. Only when the habitual patterns of therapist and patients are used as norms is it possible to assess the significance of the countertransference reactio

ABSTRACTPatients with chronic and severe personality disorders place heavy demands on Health and Social Services. In this paper we present the findings of a research study which evaluated the impact of specialized inpatient psychosocial treatment on patterns of Health Service utilization in patients referred for admission to the Cassel Hospital. The comparison between a pre‐treatment and a post‐treatment sample showed a significant decrease in the use of medical, surgical and psychiatric services in the year following discharge from the hospital. We also found that the post‐treatment group relied less on the use of psychotropic medication, on cigarette and alcohol consumption, while the employment rate was increased. ABSTRACTThis paper examines the relevance of attachment theory in the provision of psychological support to people who are affected by HIV. It highlights the themes of change, loss and vulnerability as central to the issues which confront members of this client group. Indications are provided of a range of attachment‐related behavioural responses which can be activated by these conditions, and illustrations are provided of how the activation of attachment behaviour can destabilize the relationships of some clients who are anxiously attached. The concept of a secure base, a term first coined by Mary Ainsworth and developed as a theme by the originator of attachment theory, John Bowlby, is presented as being a central provision by the therapist in work with clients who are affected by HIV, and illustrations are given of how the therapist might begin to help build a secure base for clients who have anxious, internal working models of attachment.
