ABSTRACTThe very first moments of contact with a patient may provide a vital clue to the core dynamic of the therapeutic process.Some analysts have stressed the importance of the first dream of an analysis. Others have focused on the first session as being seminal. In this paper I want to raise a few questions about what goes on in the very first moments of meeting a patient and its potential implications for the subsequent work. I shall try to differentiate the nature of what occurs and suggest that such differentiations might contribute to assessment, particularly where the initial interview is also the first session with the same therapist.

ABSTRACTA well‐established sense of gender does not depend on sexual object choice but on a sense of security in belonging to the gender correlated to one's biological sex. Cross‐gender identifications are often found in heterosexual couples who show a marked reversal of conventional gender norms and whose typical interaction constitutes a shared defence against their mutual lack of gender security. Both partners are ‘possessed’ by the contrasexual archetype (anima and animus) and have been unable to integrate their contrasexual potential. Three aetiological factors seem to predispose both men and women towards cross‐gender identification: an over‐close relationship with the parent of the opposite sex, a negative identification with the parent of the same sex and inappropriate use by their mothers as substitute caretakers (‘narcissistic objects’). ABSTRACTWhen a therapist is bored in the clinical session this is always, unless pathological, a countertransference issue. Understanding the causes and meanings of this boredom thus adds to an understanding of the therapeutic process and relationship. It may have a variety of causal factors which need teasing out in the furtherance of the therapeutic process. Boredom may be connected with feelings of emptiness or drowsiness but is in itself a distinct phenomenon.

ABSTRACTIn this paper narcoleptic states refer to states of drowsiness or sleep that occur both within the patient and the psychotherapist during a psychoanalytic psychotherapy session.
