This review assesses the current position of desflurane and sevoflurane, two new volatile anaesthetic agents. Sevoflurane is currently licensed for clinical use in Japan. Desflurane is undergoing phase 2/3 clinical trials in Europe and the USA. The properties of the agents are reviewed and their potential role in the future is discussed.

New information about the supraspinal and spinal mechanisms of opioid analgesia and developments in the field of opioid receptors are reviewed. The physiological role of the endogenous opioid peptide system in modulating behavioural responses to stress, in epilepsy, and in the regulation of cardiovascular function is also discussed.

Further developments in the use of computer-controlled infusions to deliver target plasma opioid concentrations are examined. Evaluations of alternative routes for opioid administration are reviewed. Opioid-induced respiratory depression has once again been reported and a commentary on the comparative studies designed to reduce this complication is provided.

Intravenous patient-controlled analgesia (PCA) use has become widespread over the past 10 years. Despite its popularity, the variables of PCA have never been carefully studied. However, recent studies indicate that a dose of 1 mg morphine gives the best pain relief with the least side effects when compared with 0.5 mg and 2 mg doses. When a concurrent continuous infusion (CD is used, the size of the demand dose should be greater than 0.4 mg. The studies also call into question the efficacy of the concurrent infusion to improve analgesia and whether PCA is better than Cl alone. Studies also indicate that PCA is safe in pediatric patients and when administered subcutaneously. Use of patient-controlled epidural analgesia (PCEA) has also been described, although the safety of PCEA administration of local anesthetics is questioned.

Opioid agonists-antagonists are defined as opioids with mixed actions. These drugs include agents with agonist-antagonist properties (substances such as nalorphine or pentazocine that appear to be agonists at some receptors and antagonists or very weak agonists at others) and the partial agonists (such as buprenorphine). This article is a current review of recent studies investigating agonist-antagonists for use in clinical medicine.

摘要:
This review covers literature relating to the use of spinal and epidural opioids. The analgesic effect of epidural and spinal opioids is compared with analgesia obtained from other routes of administration. Means of enhancing the analgesic effect with other drugs—most importantly the α2-agonists—and preventing side effects with opioid antagonists are considered. The review also draws attention to the use of pethidine as a local anaesthetic and notes reports of complications from epidural or spinal administration of opioids.

The use of non-steroidal anti-inflammatory drugs (NSAIDs) for the perioperative management of pain is increasing. Newer drugs are being developed and there is a greater understanding of their mode of action. We review work which assesses their efficacy as analgesics in a range of surgical procedures and examine the problem of side effects.
