摘要:
Traditional methods of measurement of respiratory function in infants, such as whole body plethysmography, have been difficult to apply in clinical environments. Newer techniques for measuring lung mechanics, such as the use of forced exploratory flow-volume curves, can be easily used at the bedside and international standards are being produced to ensure maximum accuracy and reproducibility. Although continuous measurements obtained during tidal breathing are affected by many variables, there is increasing interest in the use of these bedside measurements of pulmonary function in clinical management and epidemiological studies.

With advances in resuscitation of the newborn and antenatal diagnosis, more infants with complex diseases are being managed by the anesthesiologist and surgeon. A review of the general management of all newborns in the operating room is presented, as well as perioperative care of infants with congenital diaphragmatic hernia and the management of infants at risk of postoperative apnea.

Studies of pediatric cardiopulmonary disease demonstrate significant advances in medical and surgical therapy. Of special interest are studies evaluating echocardiography (fetal and neonatal) interventional cardiology, changes in surfactant associated with cardiopulmonary bypass, and alterations in cerebral perfusion during low-flow or circulatory arrest states. This review also reflects the prevailing interest in preserving myocardial perfusion and function and in improving outcome after heart transplantation.

The changing practice of anesthesia has included an increased use of anesthesia services outside the traditional operating room environment. In order to provide quality service and patient care in these settings, anesthesiologists must understand the unique problems of providing anesthetic services in new settings. This involves inspecting and preparing the area, as well as establishing protocols for such diverse subjects as patient selection, pre-anesthetic recovery, and patient follow-up. This is an exciting opportunity for anesthesiologists to expand the scope of their practice.

The proportion of patients admitted for surgery under anaesthesia that are elderly, psychiatrically ill or have severe drug addiction, is increasing. In these groups of patients, the perioperative risks are increased because of the overall reduction in the functional reserve of the various systems and because of drug interactions. Relevant factors that affect the response to surgery and anaesthesia will be highlighted.
