Interventional techniques such as nerve blocks, surgery and stimulation modalities for pain management are increasingly important because of their effectiveness and potential cost savings. They are applied to patients after thorough evaluation has clarified the cause(s) of the pain. Health-care professionals must understand the risks, benefits, and capabilities of such techniques to appreciate most fully how to use them for the benefit of the patient.

Reviewed in this article are recent publications on the role of psychosocial factors in chronic pain. The material is organized around three approaches: psychiatric symptom, operant-behavioral, and cognitive-behavioral. The first approach continues to generate many assessment studies, but few studies involving treatments. The second remains influential, but has become increasingly merged with the third. The third is reflected in the high volume of current research into patients' pain beliefs, understanding and coping skills.

Multiple interventions have been used in the rehabilitation of chronic pain patients. There is a paucity of research into the efficacy of rehabilitation techniques in the management of chronic pain. Appropriate and detailed criteria for assessing the validity and applicability of clinical research have been proposed. Recent controlled studies suggest that stretching exercises are of benefit in patients with chronic low-back pain. A controlled trial of transcutaneous electrical nerve stimulation (TENS) demonstrated no clinically or statistically significant effect of TENS over placebo in chronic low-back pain. Controlled studies using laser treatment of chronic orofascial pain conditions revealed no statistical difference between the laser and placebo treatment. Comprehensive multidisciplinary pain treatment is effective in increasing physical function, decreasing medication usage and increasing the return-to-work rate for patients with chronic pain problems.

The crux of cancer pain management is that the individual needs of each patient must be met in terms of both the time and flexibility of the wide variety of treatment modalities that can be considered useful. The foregoing papers emphasize individualization of therapy; development of appropriate expertise to undertake specific treatment modalities; ongoing assessment of the intensity of the pain and its consequent effects on the patient's psyche and family members; and, lastly, always using what is expeditious, simple and economically advantageous.
