The laryngeal mask airway has gained widespread acceptance as a general purpose airway for routine anaesthesia and a number of specialized uses have emerged. Evaluation of recent publications on the laryngeal mask airway indicate that this airway has gained widespread acceptance and is the preferred device in a number of situations.

摘要:
The aquired immune deficiency syndrome pandemic has altered transfusion practices. Indications for red blood cells and other components have changed; red cell substitutes are being actively pursued and substances that stimulate production of formed elements have been introduced into clinical practice. Pharmacologic agents designed to decrease surgical blood loss are being used in a variety of procedures. The effect of allogeneic blood on the immune system is a matter of significant concern. Although a zero-risk blood supply is probably not achievable, it is essential that patients are not denied life-saving therapy because of unrealistic fears about complications of transfusion.

摘要:
Allergic and pseudoallergic reactions that occur under anesthesia represent a significant cause of perioperative morbidity and mortality. Histamine has been implicated to be one of the major mediators of such reactions. Recent studies have demonstrated that perioperative histamine release may occur more frequently and with less predictability than previously thought. Rash and hypotension may be important signs of these reactions to anesthetic drugs and adjuvants. However, present evidence suggests that H1- and H2-receptor blockade may be effective in preventing histamine-related symptoms.

Patients for vascular surgery present challenges because of a high prevalence of cardiac disease. Studies to improve precision of diagnostic tests of predictors of adverse cardiac events should result in better management. For the abdominal aortic aneurysm, greater understanding of the pathophysiology of cross-clamping and unclamping should lead to appropriate measures to physiologic disturbances. For thoraco-abdominal aortic aneurysms, strategies reported may be effective and decrease the incidence of neurologic complications. For peripheral vascular surgery, the Perioperative Randomized Anesthesia Trial studies indicate an advantage for epidural anesthesia in lower limb revascularization. The risk of epidural hematoma is small but real. Carotid endarterectomy under regional anesthesia has shown to be satisfactory in many reports, whereas general anesthesia offers patients more comfort and a controlled airway. Further work is required for a better understanding of the mechanisms of vascular diseases, effects of surgical interventions, and different anesthetic managements, which would provide a rationale for effective treatments and improve outcome.

摘要:
Liver transplantation with reduced-size organs was developed to alleviate the mortality resulting from the scarcity of suitable cadaveric grafts. Available techniques include reduced-size cadaveric grafts, split grafts, and liver grafts from living relatives. These techniques yield results comparable to or even better than full liver allografting, but should be restricted to experienced and well organized liver centres. The anaesthetic management of paediatric liver transplantation differs from that in adults because of the paediatric physiology and the differences in the surgical procedures. Although a standard regime does not yet exist, the preoperative preparation, monitoring, induction and maintenance of anaesthesia, and intraoperative considerations, are emphasized.
