Despite the technical advances and the great amount of research on upper airway management in recent years, improved patient outcome has not been demonstrated. This article reviews recently reported experience with the flexible fiberscope, the laryngeal mask airway (LMA) and percutaneous dilational tracheostomy as well as studies on functional airway anatomy and the forces applied at direct laryngoscopy. The American Society of Anesthesiologists (ASA) Task Force algorithm for management of the difficult airway is discussed.

摘要:
A full and detailed preoperative assessment is essential before any patient receives an anaesthetic. Preoperative preparation should be regarded as of similar importance to the conduct of the anaesthetic itself. Much interest has centred around the value of the preoperative investigations and the current consensus of opinion is that there should be a positive indication for every investigation — nothing should be regarded as routine. The traditional period of fasting has been re-examined and it is likely that the time-honoured rules will be rewritten. Patients will probably be allowed some access to clear fluids until 2–3 h preoperatively in the future. Evidence is accumulating that preoxygenation should probably be routine for all elective patients and not just the emergency and special cases. The search continues for the ideal premedicant. The new oc2-agonists may have a role to play, but the evidence is so far incomplete.

摘要:
Examination of a year's literature in this field demonstrates no major advances. In what is of necessity a varied selection of topics there are some common threads. For improvements in the provision of an anaesthetic service we now have to look to large scale projects, both in literature reviews and clinical trials. This is evident in papers on: (1) learning and consciousness during anaesthesia; (2) a multicentre trial to identify independent predictors of adverse perioperative outcomes; and (3) a discussion of research into the proposed advantages of regional over general anaesthesia.

摘要:
Many drugs used in anaesthesia and intensive care, particularly those with profound effects and short durations of action, require frequent adjustment of their rate of administration to optimize that effect. Technological advances have provided the opportunity to deliver such drugs more accurately to achieve the required clinical response. Administration of some drugs can be controlled automatically in negative feedback or closed loop fashion, by linking the monitor of the clinical effect with the pump used to deliver the drug.
