摘要:
Current investigations into pre-eclampsia centre on possible prophylaxis with low-dose aspirin. Therapy with methyldopa has beneficial effects on maternal blood pressure and also on placental perfusion. Some evidence is found for central anticonvulsant actions of magnesium sulphate. Monitoring blood pressure in pre-eclampsia using automatic oscillometry gives diastolic readings of about 10 mmHg lower than with auscultation, thus presenting diagnostic exactitude.

摘要:
Recent advances in analgesic techniques and changes in the philosophies and attitudes of parturients, midwives, obstetricians and paediatricians to the conduct of labour are of great importance to the obstetric anaesthesiologist. This team of care-providers along with the parturient herself should appreciate the incidence, significance, and implications of severe pain in labour, and the methods available for its relief. Such education should be made available to all women during pregnancy to enable parturients to give truly informed consent for analgesia or anaesthesia, which may be necessary at short notice. Over the past few years attitudes to modern obstetric care have changed, and patient-controlled techniques, especially epidural patient-controlled analgesia which requires smaller drug doses, continue to evolve. Opioids are making a comeback without significant adverse effects on neonatal welfare.

Short-term complications after epidural and subarachnoid anaesthesia include haemodynamic instability, nausea, and dural puncture. The awareness and evaluation of longer-term sequelae including headache and backache is increasing. The major problems which occur during general anaesthesia include awareness during light anaesthesia, acid aspiration, and the hypertensive response to intubation.

摘要:
The search continues for improved methods of detection of fetal distress. This review discusses the use of cord blood erythropoietin, umbilical artery blood velocity waveform, continuous fetal heart rate monitoring, and fetal biophysical profile scoring in the detection of fetal distress. Also discussed are a number of situations where pregnancy is complicated by either obstetric conditions such as breech presentation, vaginal birth after previous caesarean section, or accompanying systemic disease, including previous organ transplantation, cardiac disease, and substance abuse among parturients.
