The effects of pregnancy on the systemic toxicity of local anesthetics have been controversial. A recent study performed on sheep demonstrates that pregnancy does not enhance the toxicity of bupivacaine or ropivacaine, a new amide local anesthetic. Two investigations pertaining to cocaine conclude that cardiovascular pertubations resulting from the drug are greater during pregnancy and that the placental transfer of the drug occurs rapidly. The pharmokinetics of bupivacaine were studied in parturients after intrathecal and epidural administration. The long elimination half-life and high blood level noted after epidural injection reinforces the fact that patients on continuous infusions require vigilance for signs and symptoms of systemic toxicity. The safety of breast feeding after a clinical exposure to local anesthetic agents is reinforced in a recent review, and the authors of another study concluded that local anesthetics do not adeversely affect uterine blood flow or fetal wellbeing. However, some authors cautioned against the addition of epinephrine to local anesthetics used for epidural anestheisa in the setting of asphyxia. Lastly, the usefulness of warming local anesthetic solutions and the volume that patients on pre-existing continuous epidural infusions require for cesarean section are discussed.

The most important recent contribution to the literature on pre-eclampsia has been the publication of two large multicentre prospective double-blind studies on the prophylactic use of low-dose aspirin during pregnancy. The studies do not support the routine prophylactic use of aspirin in all women judged to be at risk of pre-eclampsia although women with early severe pre-eclampsia in a previous pregnancy should be seriously considered for treatment. Other aspects of pre-eclampsia/eclampsia and medical disorders in pregnancy are discussed.

Some currently available data suggest that epidural analgesia can increase the risk of cesarean delivery for dystocia. This review discusses this issue and its potential impact on obstetric anesthesia practice. New ideas about patient-controlled epidural analgesia, ambulatory epidurals, intrathecal opioids, maternal positioning, and postpartum back pain are also presented.

Caesarean section rates and both maternal and fetal outcomes are important aspects of obstetrical anaesthesia. There have been many significant contributions to the literature this year, particularly the latest triennial British survey of maternal mortality. Other aspects of anaesthesia for caesarean section have also been explored, including regional anaesthetic technique, haemodynamic studies, intraoperative blood loss and postoperative analgesia.

The recent literature has shown a significant decrease in the incidence of obstetric anaesthesia-related maternal mortality and the short-term adverse effects of traditional anaesthetic techniques such as postdural puncture headache and spinal anaesthesia-induced hypotension. However, the increased use of spinal opioids during labour has been associated with unexpected side effects such as hypotension. Finally, potential long-term sequelae of labour epidural anaesthesia and postpartum back pain continue to be discussed.
