The popularity of ambulatory (day-case) surgery has continued to grow over the past two decades. The availability of improved drugs (for example, propofol, desflurane, sevoflurane, mivacurium, alfentanil, remifentanil, ketorolac, flumazenil, ondansetron), anesthetic techniques (for example, monitored anesthetic care) and monitors (for example, electroencephalogram-bispectral device), as well as less invasive surgical procedures ('key hole' techniques, for example), has contributed to this growth. This issue of Current Opinion in Anaesthesiology reviews the recent developments in the subspecialty of ambulatory anesthesia.

Health care is in an active state of transformation and reform. Currently, it is estimated that 70-80% of surgical patients enter the hospital on the day of surgery. Providing quality, cost-effective preoperative preparation of patients undergoing anesthesia and surgical procedures is a central issue in perioperative patient management. A successful Anesthesia Preoperative Evaluation Clinic can demonstrate significant clinical advantages, improve quality and value for our customers, and provide visible leadership in responding to rapidly changing healthcare demands.

摘要:
The concept of minimally invasive surgery has been associated with a large increase in the number and type of procedures carried out in ambulatory surgical units. The caseload has given an impression to the public and to patients of a minimal level of risk. We in anesthesiology are not of the same opinion and are striving to find a balance between expectations of patients, surgical needs and what we believe is safe for patients to endure, and at the same time reassessing preanesthetic evaluation, which is the basis of optimal patient care. The challenge is to continue to decrease morbidity and mortality in the new approaches to care so that cost savings and increases in productivity do not impinge on patient benefit or comfort.

Outcome of day-case surgery is greatly affected by the eventual presence of postoperative pain and emesis. The most reasonable approach is pain prophylaxis in every patient, and prophylaxis of postoperative nausea and vomiting in patients at risk. This article reviews, in the light of recently published data, the efficacy of different prophylactic actions related to the topic. Prevention of pain is widely referred to as antinociceptive prophylaxis, which provides a decrease in postoperative pain and not to pre-emptive analgesia, although pre-emptive analgesia is discussed.

摘要:
With the increase in numbers of surgical patients who are being treated on an ambulatory basis, it is important to develop reliable criteria to identify when patients are ready to go home. A scoring system may speed up discharge. Recent evidence suggests that drinking and voiding are not mandatory discharge criteria, and insisting on them will delay the discharge of patients unnecessarily. Recent research work indicates that certain cases can bypass the Post- Anesthesia Care Unit without compromising safety. Although major complications of ambulatory surgery are rare, pain, nausea, headache, dizziness, and drowsiness are still distressingly common.
