摘要:
Carotid endarterectomy is most successful in younger patients who have >75% stenosis. Effective medical therapy includes anticoagulation, blood pressure control and normoglycemia. Isradipine is a new anti-atherosclerotic agent. Neuroradiologic techniques are increasing in number and inventiveness. Isoflurane remains the agent of choice although propofol may increase cerebral protection. Aggressive postoperative care of poor-grade aneurysm by volume expansion improves outcome.

Neurodiagnostic invasive and non-invasive procedures may be performed under sedation or general anaesthesia. Both have to meet the demands of patients with neurological pathology or elevated intracranial pressure (ICP). Paediatric patients are particularly difficult to monitor, especially in nuclear magnetic resonance imaging (NMRI).

摘要:
Therapeutic approaches to fluid loss should not only replete intravascular volume but also minimize cerebral oedema. Maintaining colloid osmotic pressure at a constant level is of minor importance in preventing the development of cerebral oedema, whereas hypertonic saline solutions may decrease brain water content. Nevertheless, this effect can only be demonstrated in uninjured brain tissue.

Drug treatment in head injury helps to control cerebral hemodynamics, cerebral perfusion pressure (CPP) and intracranial pressure (ICP). Drugs of relevance are osmotics, dopamine, barbiturates, and arteriolar and venous vasoconstrictors. Ca2+antagonists may relieve vasospasm. Although much is known about the effects of these drugs on various pathophysiological conditions, their clinical value in the treatment of head injury has yet to be elucidated.

Operative monitoring for cerebral hypoxia remains an active area of research. Material concerning the use of electroencephalography (EEG), somatosensory-evoked potentials (SSEPs), arterial-venous O2content difference (AVDO2), transcranial Doppler sonography and cerebral blood flow (CBF) are reviewed. Near-infrared spectroscopic monitoring is covered in particular detail as it is a novel approach to this problem.

Indications for and mechanisms of the action of interpleural analgesia are discussed and technical points that make this technique safer are considered in this review. Continuous intercostal nerve block, an alternative to the interpleural technique, is described. Finally, new data suggesting beneficial effects of thoracic epidural anaesthesia (TEA) in patients with coronary artery disease are reported.

Different approaches for performing lumbar or sacral plexus nerve blocks are reviewed. Lumbar nerve blocks may be achieved either by anterior or posterior approaches, while local anaesthetic solutions may be injected either in the psoas or the fascia iliaca compartment.
