摘要:
Research reported during the past year has enhanced our understanding of conditions that lead to the complex changes that are observed in hypertensive disorders of pregnancy. An association between placental pathology and a multisystem disorder that is characterized by endothelial dysfunction which involves genetic and immunological investigations has been identified. On the basis of these findings, promising screening tools for early detection of pre-eclampsia were identified. No marked changes in anaesthetic approach to hypertensive disorders of pregnancy occurred during the period of review, apart from a reappraisal of spinal anaesthesia as a safe technique in caesarean section, even in patients with severe pre-eclampsia. A multidisciplinary approach to management and therapy is needed and the right balance must be sought between the needs of the mother and baby, both of which are jeopardized by the hypertensive disorders of pregnancy.

The present review addresses several medical and obstetric factors/problems, and their interactions during pregnancy. These include the following: maternal age; morbidity and mortality during pregnancy; thromboembolism; gestational diabetes; haemolysis, elevated liver enzymes, low platelets syndrome; human immunodeficiency virus; hepatitis C; preterm labour; antenatal administration of corticosteroids; intrapartum surveillance; breech delivery; and caesarean section.

摘要:
We present a brief overview of recent literature concerning some of the drugs used in pregnancy, labour and delivery. Obstetric anaesthesia continues to evolve through the reuse of old drugs such as magnesium sulphate or the manipulation of current drugs (e.g. propofol for emesis) in order to improve patient outcome. Pregnant women have traditionally been therapeutic orphans. The use of new agents such as levobupivacaine and ropivacaine in obstetric patients lags behind that of their non-pregnant counterparts. However, this gap is decreasing and these new drugs offer benefits to the parturient woman.

摘要:
The present review addresses common and rare, but potentially severe complications of obstetric anaesthesia. Although several recent randomized and controlled studies on the prevention of spinal hypotension have been reported, the incidence of neurological complications caused by neuraxial blocks is estimated primarily from the results of retrospective investigations and case reports. In order to avoid potential risks in obstetrics, however, vigilance and close monitoring of the patient are critical. The adoption of published recommendations (e.g. those regarding regional techniques and coagulopathy, and difficult airway management) may help to improve the outcome for mother and child.

Major complications and death are rare events, but cases are reported even in connection with minor surgery. Most of the patients for ‘minor’ surgery are currently treated as day cases and during the hospital stay only the immediate or intermediate outcome is seen. The results of the evaluations on the late outcome at home can be surprising.

Up to 60% of all children undergoing anesthesia and surgery experience significant anxiety and distress during the perioperative period. Currently, both behavioral interventions such as parental presence during induction of anesthesia and pharmacological interventions such as sedative premedication are used to treat this clinical phenomenon. In this review, we examine the effectiveness of these interventions on clinically relevant perioperative outcomes.

The advantages of rapid induction of and emergence from sevoflurane anaesthesia may be more than offset by the frequent occurrence of agitation during induction and recovery, and a possible epileptogenic effect. The mechanisms and possible strategies to prevent these drawbacks are reviewed, on the basis of the most recent literature.
