Auditing of anesthesia performance has been advocated for many years; however, only now is it beginning to come into its own as a valuable method for improving patient care, resource management, resident education, and the personal performance of physicians. Although the best method for auditing is not yet clear, certain principles are common in the better ones. Recent articles show how data derived through auditing can be used effectively, particularly to improve the outcomes of anesthetic care.

Cost-effective anesthesia is necessary as medicine enters an era of managed care and cost constraints. Practical methods to achieve cost savings in anesthesia include preoperative testing; selection of anesthetic agents, narcotics and muscle relaxants, and monitors; low-flow delivery methods; regional anesthesia; reduction of wastage; and continuing education.

The elderly represent the most rapidly growing segment of our population. A review of the recent literature yields information important in the anesthetic care of this group of patients. In particular, a better understanding of the nature of morbidity and mortality following anesthetic care in elderly patients, risks relative to anesthetic technique, thermoregulation differences in this group, and pharmacology of newer anesthetic agents have been reviewed.

Minimally invasive or minimal access surgery is associated with significantly less trauma than conventional surgical access. The technology required is expensive and its adoption has major implications for patient services, hospital design, operating room equipment, surgical speciality alignment, and training. Intraoperative complications are mostly due to traumatic injuries sustained during blind trocar insertion and physiological changes associated with pneumoperitoneum and patient position.
