SUMMARYDebate about how to obtain the statistics needed to monitor the health of the population has been stimulated by a resurgence of interest, both nationally and locally, in public health. At the moment, it is clear that data systems are not up to the task. Existing national data collection systems are largely concentrated on measuring mortality and monitoring the activities of health services. Thus, work done so far has had to rely on proxy indicators of health, rather than those which would be recommended ideally. In the face of this, central government has started to discuss how to monitor and promote the health of the population. Also, over the past 10 years, health and local authorities have started to search for data to guide them in targeting the activities that they undertake to improve the health of their populations. This has highlighted the need to extend the limited range of morbidity data currently collected. Before constructing new data collection systems, which might themselves be equally inappropriate, it is essential to clarify the questions to be asked, however. There is a need to decide how concepts such as ‘health’ and ‘deprivation’ should be defined, to determine how appropriate data can be collected and to select the analytical methods to be used.

SUMMARYThe importance of general statistical principles of study design and analysis to quality‐of‐life assessment in clinical trials is emphasized. Basic methods are reviewed briefly, with reference to three examples. Careful use of standard tools supplemented with context‐specific scales is recommended. Problems of weighting and aggregation are discussed; the use of simple weighting schemes supplemented by sensitivity analysis is suggested. Some technical issues are explored, including factorial question structure, components of variance to distinguish mean treatment and patient‐specific treatment effects and informative loss to follow‐up. Simplicity of design, analysis and presentation are stressed.

SUMMARYThis paper reviews the problems that bedevil the selection of an appropriate test for the analysis of a 2 times 2 table. In contradiction to an earlier paper, the author now argues the case for the use of Fisher's exact test. It is noted thatalltest statistics for the 2 times 2 table have discrete distributions and it is suggested that it is irrational to prescribe an unattainable fixed significance level. The use of mid‐pis suggested, if a formula is required for prescribing a variable tail probability. The problems of two‐tail tests are discussed.

SUMMARYThis paper examines the recent review of the Resource Allocation Working Party formula for allocating resources to the regions in the National Health Service from which the current formula‐is derived. Increasingly public resources are allocated by means of formulae which are based on multiple‐regression techniques which attempt to model ‘need’. The paper highlights some of the statistical and methodological problems in developing such formulae using resource allocation in the National Health Service as a case study. The statistical approach of the review is subjected to a rigorous critique and ways of improving the method are suggested. The paper then explores the limitations inherent in this type of modelling methodology. It is argued that there are real problems in attempting to translate technical or scientific models into public policy. This is compounded by the fact that regression techniques are often used by policy makers to give the impression of neutrality while pursuing political ends. The fundamentally political nature of the choice of need indicator must be acknowledged.
